Krankenhauspharmazie

Preparation of emergency medication from commercially available epinephrine and norepinephrine injection solutions plays an important role in the daily workflow of many hospital pharmacies. As it is a common practice to prepare individual dilutions under aseptic conditions and as epinephrine derivatives show a limited stability, shelf life and pharmaceutical quality of the respective preparations are of general interest. Studies conducted by the pharmacy of the hospital in Cologne-Hohenlind and the University of Würzburg, Germany, confirm that dilutions from proprietary products (e. g. Arterenol®) can be prepared in advance if stored properly.

Am 23. Oktober 2009 hat die Food and Drug Administration (FDA) auf Empfehlung des Centers for Disease Control (CDC) den Neuraminidaseinhibitor (NAI) Peramivir im Rahmen einer “Emergency Use Authorization” (EUA) zur Behandlung der 2009-H1N1-Influenza (“Schweinegrippe”) in den USA zugelassen. Nach Oseltamivir (oral) und Zanamivir (inhalativ) steht nun mit Peramivir der erste NAI zur parenteralen Gabe zur Verfügung.

Die 5α-Reduktase-Inhibitoren Finasterid und Dutasterid können bei der BPH gleichwertig eingesetzt werden. Manche Leitlinien sehen allerdings α-Blocker den 5α-Reduktase-Inhibitoren überlegen. In der Studie von McConnell et al. gibt es Hinweise, dass eine Kombinationstherapie aus dem α-Blocker Doxazosin und dem 5α-Reduktase-Inhibitor Fina-sterid einer Einzeltherapie überlegen ist. Allerdings scheinen die Kosten deutlich höher als der Nutzen zu sein.
