Homeopathy and The Lancet

Introduction
The Lancet of August 27, 2005 featured a cluster of articles
highly critical of homeopathy which attracted considerable
media attention. The media reports echoed The Lancet’s press
release: ‘homeopathy is no better than placebo’. The center-
piece was a meta-analysis of clinical trials of homeopathy
compared with clinical trials of allopathy (conventional
medicine) (1). The first author is Aijing Shang, but the leader
of the research group is Prof. Matthias Egger of the Depart-
ment of Social and Preventive Medicine, University of Berne,
Switzerland.
The meta-analysis formed part of the Complementary
Medicine Evaluation Programme (Programm Evaluation
Komplementa
¨rmedizin, PEK) financed by the Swiss Federal
government. The international review board of PEK has
publicly protested at political interference in the scientific
process: ‘There is a consensus among the review board
members that the final PEK process deviated from what
would have been expected by conventional standards. Espe-
cially disconcerting was the fact that the products of the
PEK process—health technology assessment (HTA) reports,
single description of studies, manuscripts for publication and
the condensed final report—were sent to the board members
but no discussion, comment, or review was solicited by the
responsible agencies’ (2).
The meta-analysis was accompanied by a short, anonymous
editorial entitled ‘The end of homoeopathy’ calling for
‘doctors to be bold and honest with their patients about homeo-
pathy’s lack of benefit, and with themselves about the failings
of modern medicine’ (3); and a more thoughtful commentary
from the Dutch epidemiologist Jan Vandenbroucke, reflecting
on the ‘growth of truth’, including the relationship between
bias, background knowledge and the concordance of clinical
results with laboratory science findings (4). Vandenbroucke
concludes that the proof of the pudding is in the eating: the
ultimate proof of the validity of a scientific or medical idea
is extent to which it changes reality.
The same issue of The Lancet featured a leak of the World
Health Organisation’s (WHO) draft report on homeopathy.
The WHO document was apparently leaked to The Lancet by
Dutch and Belgian doctors hostile to homeopathy; their com-
ments and the (hostile) comments of Prof. Edzard Ernst of
the University of Exeter were published. Dr Xiaorui Zhang,
Traditional Medicine Coordinator of WHO, who is responsible
for the report, was also interviewed, but declined to comment
on a leaked, confidential draft. This leak came only 2 days
after The Times of London published, as its front page lead, a
remarkably similar story: a leak of the Smallwood Enquiry
on The Role of Complementary and Alternative Medicine in
the NHS commissioned by The Prince of Wales’ Foundation
for Integrated Health. It is ironic that the editor of The Lancet,
Dr Richard Horton, wrote to The Times accusing Prof. Ernst of
having ‘broken every code of scientific behaviour’ for leaking
the draft report of the Smallwood Enquiry (and incidentally
describing complementary medicine as ‘a largely pernicious
influence... preying on the fears and uncertainties of the
sick’), while simultaneously doing the same to the WHO
report in his own journal!
Dr Horton also wrote an open letter to the UK Secretary
of State for Health, Patricia Hewitt and the Chairman
of the National Institute for Clinical Excellence (NICE)
Prof. Sir Michael Rawlings, calling for the use of homeopathy
in the NHS to be reviewed in light of this publication.
The Meta-Analysis
The meta-analysis at the centre of the controversy is based
on 110 placebo-controlled clinical trials of homeopathy and
110 clinical trials of allopathy (conventional medicine), which
are said to be matched. These were reduced to 21 trials
of homeopathy and 9 of conventional medicine of ‘higher
quality’ and further reduced to 8 and 6 trials, respectively,

which were ‘larger, higher quality’. The final analysis which
concluded that ‘the clinical effects of homoeopathy are
placebo effects’ was based on just the eight ‘larger, higher
quality’ clinical trials of homeopathy. The Lancet’s press
release did not mention this, instead giving the impression
that the conclusions were based on all 110 trials.
The criteria for the matching of the homeopathic and con-
ventional trials were not clearly stated, and it is evident from
the numbers above that the clinical trials of homeopathy and
conventional medicine were not, as claimed, well matched—
the homeopathic trials were generally of better quality. And
this is a crucial parameter [as Shang et al. (1) acknowledge];
it is well established that high quality trials are less likely to
be positive than those of lower quality. Because of the
individualization involved, it is difficult to do large-scale trials
of homeopathy (again this is evident from the figures
above), so the final sample is unlikely to be representative of
trials of homeopathy. An example is a study that may have
been included in the final eight, which looked at the use of a
homeopathic medicine for prophylaxis of influenza, an indica-
tion for which it is not recommended, and obscured the identity
of the homeopathic medication (5). However, a Cochrane
Review has concluded that it is probably effective for treat-
ment (as opposed to prevention) of flu-like illnesses (6). A
number of similar instances could be cited, but since we do
not know which studies are under discussion, there is little
point.
Transparency, Sensitivity and
External Validity
One of the most serious criticisms is the complete lack of
transparency: we have no idea which eight trials were included
in the final, damning, analysis. The literature references are
not given, nor any information on the diagnoses, numbers
of patients, etc., nor can these be deduced from the article.
Prof. Egger has refused several requests to disclose the iden-
tity of the eight trials. This is not even a matter of scientific
method, but of natural justice: the accused has the right to
know the evidence against him.
Meta-analysis should incorporate sensitivity analysis. In this
case the obvious sensitivity analysis is to look at the 21 trials of
‘higher quality’, particularly since it appears that the criterion
‘larger’ appears to have been added retrospectively to ‘higher
quality’. But the result of this analysis was not published.
This meta-analysis assessed quality purely in terms of
internal validity. Internal validity is the extent to which a trial
measures what it purports to measure; external validity the
extent to which what it measures is meaningful in the real
world. This is an important concept, best explained by an
example. A clinical trial of homeopathy in childhood asthma
by White et al. (7), published in Thorax in 2003, was reported
as showing that homeopathy is ineffective. This trial was
of high internal validity because, among other things, it used
a predefined primary outcome measure: quality of life as
measured by the childhood asthma questionnaire.
The results showed no difference in the quality of life scores,
although the secondary outcome measures (such as severity
and time lost from school), consistently favored homeopathy
over placebo. However, because quality of life was the primary
outcome, the authors reached a negative conclusion. But, as
was pointed out several times in the ensuing correspondence,
this conclusion was meaningless because of a ‘ceiling effect’:
the children had normal quality of life when they entered the
study, and this could not have been expected to improve fur-
ther! (8–11) This study was of high internal validity but very
low external validity, but would have been classified ‘high
quality’ in The Lancet meta-analysis.
Upper Respiratory Tract Infections
Shang et al. (1) state that ‘eight trials of homoeopathic reme-
dies in acute infections of the upper respiratory tract ... indi-
cated a substantial beneficial effect ... sensitivity analyses
might suggest that there is robust evidence that the treatment
under investigation works. However, the biases that are preva-
lent in these publications, as shown by our study, might pro-
mote the conclusion that the results cannot be trusted’. They
state that eight studies is too few to question their conclusion
about the whole set of publications. Their conclusion about
the whole set, however, was also based on eight studies. Is
eight enough for a conclusion or not? Or does it depend on
what that conclusion is?
But perhaps the most telling single criticism of this
meta-analysis is that it fails, on multiple counts, to meet
the generally accepted standards for meta-analysis—the
QUOROM statement (Quality of Reports of Meta-Analyses
of Randomised Controlled Trials), published in The Lancet
itself in 1999 (12). The main failings are those outlined
above, although there are others.
The implausibility of homeopathy is an important Bayesian
prior for Shang et al. (1), but they fail to quote emerging evi-
dence for in vitro activity of ultramolecular dilutions (13,14)
which has important implications for the implausibility of the
claims made for homeopathy.
Conclusion
This meta-analysis is subject to fundamental criticisms.
Regrettably, the media have already reported The Lancet’s
version of the story. Homeopathy’s popularity is growing
worldwide despite many such attacks. To paraphrase Mark
Twain, reports of the death of homeopathy are much exagger-
ated: the facts simply are incompatible with The Lancet’s
claim that the end of homeopathy is nigh. Regrettably, this
attack will only widen the divisions. The way forward is
open, transparent science, not opaque, biased analysis and
rhetoric.

Background Even though several initiatives have been undertaken in different locations worldwide to collect clinical data in homeopathy, it is important to further investigate these aspects in the context of health care in India. Objective The study aimed to gather and analyze patients' clinical data and to derive insights into homeopathic treatment using an internet-based software program for data storage, retrieval and repertorization. Methods A multi-center observational study was conducted across 14 homeopathy outpatient clinics in India that are affiliated with the Central Council for Research in Homoeopathy (CCRH). Patient symptoms and demographic details were documented anonymously, and prescriptions were guided by repertorial suggestions from the Vithoulkas Compass software. During follow-up visits, treatment outcome was also recorded using an online assessment form. A retrospective analysis of data on patients' demographics, follow-up visits, morbidity (International Classification of Diseases 11th Revision), rubrics used, prescribed medicines and the level of improvement was achieved using Microsoft Excel-generated pivot tables. Results Throughout the study duration of one year a total of 2,811 patients attended the 14 outpatient clinics, of whom 2,468 were new patients with a total of 2,172 initial homeopathic prescription entries. Across the study, there were 3,491 prescriptions and 1,628 follow-up consultations for 868 follow-up patients, all of which data were thoroughly analyzed. The highest frequency of patients was in the 20–49 age group, and a higher proportion of the patients overall was female. Musculoskeletal, dermatological and respiratory complaints were the most frequently reported. The rubrics “Desire for sweets” and “Desire for spices” emerged as the most commonly used in the repertorizations. Further, Sulphur stood out as the most commonly prescribed medicine overall. With homeopathic treatment, some degree of clinical improvement was reported in 86% of the follow-up cases. Conclusion Homeopathy is prescribed in CCRH outpatient clinics for a wide range of ailments in people across India, with at least some clinical improvement noted in a high proportion of those patients. The large-scale systematic data collection in these clinics has provided clear insights into the use and clinical value of homeopathy in India, with the potential to build a substantive nationwide data inventory over time.

El presente artículo explica y ejemplifica la definición de pensamiento crítico en medicina y su relación con el modelo científico. Pensamiento crítico, o mejor aún, cognición educada, consiste en saber identificar un conocimiento basado en evidencias de otro que no lo es, en saber gestionar la incertidumbre en situaciones como la aún actual pandemia de COVID-19. Consiste en buscar la veracidad como método. Y requiere una capacidad de crítica con dosis de escepticismo. La segunda parte del artículo argumenta la importancia de tener conocimientos basados en modelos. Conocimientos que no creencias. El modelo científico consiste en enmarcar el conocimiento científico en un modelo explicativo que no un modelo mágico ni un modelo justificativo. El modelo científico reconoce sus fortalezas y debilidades, es decir, se aplica de manera crítica, y está obligado a explicar y predecir parcelas de la realidad física, psíquica o social. En general se asocia el pensamiento crítico a un camino para llegar a la verdad, pero, tal como describimos en el presente artículo, resulta mucho más apropiado considerarlo un camino para rectificar y perfeccionar nuestros hábitos motores y de reflexión, y una metodología para pensar conforme a modelos que expliquen lo mejor posible la realidad y para rectificar sin apenas dolor moral.

Résumé Les médecines alternatives et complémentaires (MAC) sont de plus en plus prégnantes sur la scène médicale française mais aussi mondiale. Cependant, elles font l’objet de questionnements légitimes quant à leur définition et leur place dans l’arsenal thérapeutique des soins en oncologie. Cet article a pour objectif de faire l’état des lieux des médecines alternatives et complémentaires reconnues par le conseil de l’ordre des médecins, et celles émergentes ou implantées dans les pratiques en France. Nous rappelons leurs fondements, leurs statuts et leurs modalités d’exercice. Une synthèse de la littérature abordant les principaux essais cliniques de qualité ayant inclus des patients atteints de cancer est réalisée. Enfin, nous aborderons des hypothèses concernant l’engouement pour ces pratiques, les conflits qui les entourent et nous tenterons d’élaborer des voies de convergence entre médecine alternative et complémentaire et médecine conventionnelle.

In 2009, the UK House of Commons Science and Technology Committee (STC) conducted an ‘evidence check’ on homeopathy to evaluate evidence for its effectiveness. In common with the wider literature critical of homeopathy, the STC report seems to endorse many of the strong claims that are made about its implausibility. In contrast with the critical literature, however, the STC report explicitly does not place any weight on implausibility in its evaluation. I use the contrasting positions of the STC and the wider critical literature to examine the ‘implausibility arguments’ against homeopathy and the place of such arguments within evidence-based medicine (EBM). I argue that the STC report undervalues its strong claims about the mechanistic plausibility of homeopathy because it relies on a misunderstanding about the role of mechanistic evidence within EBM. This is not a conclusion for a revision of the role mechanistic evidence plays within EBM, however. It is a conclusion about the inconsistency of the STC report’s position towards implausibility arguments, given the evidential claims they endorse and the atypical situation that homeopathy presents. It provides a further example of the general point that mechanistic reasoning should not be seen as providing categorically lower quality evidence.

This report on the current state of homeopathic research provides a summary on the research areas of healthcare research, randomised controlled clinical trials, meta-analyses and basic research. It aims to contribute to the discussion within the field of homeopathy concerning the need for research, the relevance of individual research fields and methods and their role in future research strategies. A summary analysis of the clinical research data offers sufficient evidence of the therapeutic effectiveness of homeopathic treatment. The results from numerous placebo-controlled trials and basic research experiments suggest, moreover, that potentised medicines offer specific efficacy. Put in perspective, there are many important open research areas – notably: Basic research into the optimisation of laboratory models and the understanding of the mode of action, Independent replications of studies in clinical and basic research, Exploration of the provision of homeopathic care in reality, also combined with conventional medicine and Health economic analyses to evaluate the costs and benefits (cost effectiveness).

Homeopathic medicine is a controversial system of medicine that has been used worldwide for over 200 years. Recently, several governments, in part, owing to government-funded reviews of research on homeopathic medicine, have stopped reimbursements for homeopathic medicines and have discouraged their use by medical professionals. This review critically evaluates four government-funded reviews of clinical research on homeopathic medicine. An analysis of government-sponsored reviews of clinical research on homeopathic medicine was conducted, including two studies from Switzerland, one from England, and one from Australia. Three of the four government-funded reviews were critical of homeopathy, claiming that there was no reliable evidence that homeopathic medicines were effective. Three of these reviews had significant flaws, with potential ethical concerns raised in one of the reviews. The most comprehensive review of homeopathic research, including analysis of clinical and basic science concerns, found the most positive results for homeopathy.

Critics of traditional and complementary medicine assert that people are putting their trust in the hands of incompetent and poorly regulated practitioners who are deliberately misleading patients about benefits leading to risks to health, and even lives. Injury or infection from acupuncture needles, severe injuries from osteopathy or chiropractic manipulation, or rejection of effective conventional medicine, are just some of the threats described for humans who use traditional and complementary medicine. The two most commonly cited safety concerns are examined: adverse drug reactions from herbal medicine and the potential for harm from delayed access to conventional care when using homeopathy. Through examination of real world scenarios and data, discrepancy between the rhetoric and the reality of these contentious issues is revealed.

Abstract: This publication aims at introducing some issues related to the scientific status of homeopathy and the critical discourses that questioning their validity. The most popular skeptical authors were identified: the main topics in their discourses were epistemology, anthropology of religion and natural sciences. Much of the content of arguments was rhetorical in nature and in derogative tone. Finally, the author presents a critical appraisal of the main arguments against homeopathy.

Die Homöopathie ist eine Therapiemethode, die seit mehr als 200 Jahren praktiziert wird und eine beträchtliche Zahl an Heilungserfolgen vorzuweisen hat. Die Evidenzlage hinsichtlich der klinischen Forschung zur Homöopathie ist nach strengen Kriterien mindestens als ambivalent zu beurteilen und weist eine stark positive Tendenz auf, wenn im Rahmen von Meta-Analysen nicht nur einige wenige Studien berücksichtigt werden, sondern ein sinnvolles Gleichgewicht zwischen Selektionskriterien und Aussagekraft geschaffen wird. Die Gründe für die Verzerrungen bei der Beurteilung des Phänomens Homöopathie sind auf der wissenschaftlichen Paradigmenebene zu suchen und wurzeln letztlich in spezifischen erkenntnistheoretischen Vorannahmen, die in ihrer unreflektierten Form als Weltanschauung angesprochen werden können. Diese Weltanschauung kann in als eine Form des reduktionistischen Materialismus identifiziert wer-den, welcher in Verbindung mit einem unkritischen Szientismus dafür genutzt wird, Erkenntnissi-cherheit und eine wissenschaftlich methodische Fundierung zu prätendieren. Dieser Komplex erweist sich im Rahmen einer epistemologischen Analyse als transzendentalphilosophisch haltloses metaphysisches Konstrukt, welches keinesfalls dazu geeignet ist, als Kriterium für Wissenschaftlichkeit zu fungieren, oder gar a priori über die Möglichkeit respektive Unmöglichkeit bestimmter Phänomene zu befinden. Der unkritische Umgang eines Teils der Homöopathiekritik mit den eigenen epistemologischen Voraussetzungen führt somit zu der paradoxen Situation, dass unter Berufung auf wissenschaftliche Grundsätze zentralen Postulaten der wissenschaftlichen Methode eine Absage erteilt wird. Vor dem Hintergrund der Science Studies lässt sich konstatieren, dass die Homöopathie nicht nur mit epistemologischen Setzungen in Konflikt gerät, sondern dass naturwissenschaftliche Grundsätze und die Methodik der klinischen Forschung in ein soziales Gefüge eingebettet sind, in welchem neben statistisch auswertbaren Daten ebenso die Denkgewohnheiten und Erwartungshaltungen von Peer-Review-Gutachtern und die hiermit in Zusammenhang stehenden Publikationsmöglichkeiten eine Rolle spielen. Nicht zuletzt ist an dieser Stelle zu konstatieren, dass Wissenschaft als soziales System über mannigfaltige Überschneidungen und Verquickungen mit anderen Systemen verfügt, zu denen sicherlich auch die Ökonomie gehört. In dieser Hinsicht ist es durchaus denkbar, dass eine wissenschaftliche Ächtung der Homöopathie mit bestimmten wirtschaftlichen Interessen konform geht, wenn sie nicht sogar teilweise durch solche motiviert sein sollte. Abschließend werden mit der Kategorienlehre, der Semiotik und den wissenschaftstheoretischen Überlegungen des amerikanischen Philosophen Charles Sanders Peirce Ansätze präsentiert, welche die aufgezeigten Aporien des reduktionistischen Materialismus vermeiden. Es wird ein begriffliches Instrumentarium eingeführt, mit dessen Hilfe sich die entsprechenden Verkürzungen bei der Interpretation des Phänomens Homöopathie klar benennen und auch umgehen lassen, ohne auf unzulässige metaphysisch aufgeladene Theoreme zu-rückgreifen zu müssen. Die Homöopathie steht innerhalb der vorliegenden Analyse stellvertretend für sämtliche Gegen-standsbereiche, die sich einer einfachen Deutung durch materialistisch-reduktionistische Erklä-rungsmuster im dargestellten Sinne entziehen. Insgesamt lässt sich die Schlussfolgerung ziehen, dass die Grenzen gängiger Erklärungsmodelle und erkenntnistheoretischer Paradigmen häufig anhand von wissenschaftlichen Anomalien erkennbar werden und dann einer umfassenden Revision unterzogen werden sollten, um global neue Zugänge und Perspektiven für die Wissenschaft zu erschließen, die nicht den Beschränkungen einer dem Fortschritt hinderlichen Weltanschauung unterliegen.

The Quality of Reporting of Meta-analyses (QUOROM) conference was convened to address standards for improving the quality of reporting of meta-analyses of clinical randomised controlled trials (RCTs). The QUOROM group consisted of 30 clinical epidemiologists, clinicians, statisticians, editors, and researchers. In conference, the group was asked to identify items they thought should be included in a checklist of standards. Whenever possible, checklist items were guided by research evidence suggesting that failure to adhere to the item proposed could lead to biased results. A modified Delphi technique was used in assessing candidate items. The conference resulted in the QUOROM statement, a checklist, and a flow diagram. The checklist describes our preferred way to present the abstract, introduction, methods, results, and discussion sections of a report of a meta-analysis. It is organised into 21 headings and subheadings regarding searches, selection, validity assessment, data abstraction, study characteristics, and quantitative data synthesis, and in the results with "trial flow", study characteristics, and quantitative data synthesis; research documentation was identified for eight of the 18 items. The flow diagram provides information about both the numbers of RCTs identified, included, and excluded and the reasons for exclusion of trials. We hope this report will generate further thought about ways to improve the quality of reports of meta-analyses of RCTs and that interested readers, reviewers, researchers, and editors will use the QUOROM statement and generate ideas for its improvement.

Homeopathy is frequently used to treat asthma in children. In the common classical form of homeopathy, prescriptions are individualised for each patient. There has been no rigorous investigation into this form of treatment for asthma. In a randomised, double blind, placebo controlled trial the effects of individualised homeopathic remedies were compared with placebo medication in 96 children with mild to moderate asthma as an adjunct to conventional treatment. The main outcome measure was the active quality of living subscale of the Childhood Asthma Questionnaire administered at baseline and follow up at 12 months. Other outcome measures included other subscales of the same questionnaire, peak flow rates, use of medication, symptom scores, days off school, asthma events, global assessment of change, and adverse reactions. There were no clinically relevant or statistically significant changes in the active quality of life score. Other subscales, notably those measuring severity, indicated relative improvements but the sizes of the effects were small. There were no differences between the groups for other measures. This study provides no evidence that adjunctive homeopathic remedies, as prescribed by experienced homeopathic practitioners, are superior to placebo in improving the quality of life of children with mild to moderate asthma in addition to conventional treatment in primary care.

Ultra-high dilutions of lithium chloride and sodium chloride (10−30gcm−3) have been irradiated by X- and γ-rays at 77K, then progressively rewarmed to room temperature. During that phase, their thermoluminescence has been studied and it was found that, despite their dilution beyond the Avogadro number, the emitted light was specific of the original salts dissolved initially.
