Human Factors in the Development and Implementation of Telerehabilitation Systems

Telemedicine services must be designed and implemented with the users in mind. When conducting telerehabilitation, factors such as age, education and technology experience must be taken into account. In addition, telerehabilitation must also accommodate a range of potential patient impairments, including deficits in language, cognition, motor function, vision and voice. Telerehabilitation technology and treatment environments should adhere to universal design standards so as to be accessible, efficient, usable and understandable to all. This will result in improved access to a wider range of telerehabilitation services that will facilitate and enhance the rehabilitative treatment and recovery of people living with varying levels of injury, impairment and disability.

Introduction
Telemedicine services usually involve a patient, a clinician,
some form of medical information and a method of data
transmission. While the details (e.g. the diagnosis of the
patient, the specialty of the clinician[s], the content and
format of the medical information, and how and when the
information is transmitted) will vary depending on the
application, there are high-level goals such as patient
satisfaction, security of medical information, device
usability and clinician adoption which are common to all
telemedicine services.
In studying the human factors of telemedicine, it is
helpful to use a macroergonomic approach. Unlike
microergonomics, which considers only the users and their
relationship with technology, macroergonomics is a
top-down approach which considers the human
technology relationship relative to the organizational
structure and external environment in which it occurs.

By
focusing equal attention on the four subsystems (personnel,
technology, organization and environment) (Figure 1),
valuable insight can be obtained into how telemedicine
programmes operate and how they can be optimized.
An important human factor in telemedicine is the role
that personnel play in all stages of planning, training and
implementation. In telemedicine programmes, the
definition of personnel (or ‘users’) is broad and
encompasses the entire range of people involved in a
telemedicine encounter, which may consist of primary care
and specialist clinicians, patients, administrators and
support staff such as technical support personnel and
caregivers.

Depending on the application, the number of
users during a telemedicine encounter could be as few as
two (e.g. a patient at home connected to a nurse at a
monitoring centre) or as many as a dozen or more (e.g. a
specialist consultation linking people at multiple sites).
Each user’s response to telemedicine will be affected by his
or her own level of technical expertise, physical capabilities
and organizational cultures.
Telerehabilitation
Telerehabilitation refers to the delivery of medical
rehabilitation services and the support of independent
living, using telehealth technologies.

It involves clinical
disciplines such as speech-language pathology,
occupational and physical therapy, neuropsychology,
assistive technology and cardiac and vocational
rehabilitation. Telerehabilitation patients include those
with stroke, brain or spinal cord injuries, amputation and
orthopaedic or developmental impairments. The types of
telerehabilitation interventions that can be delivered are
broad and include applications such as direct patient service
delivery,

specialist consultation,

remote monitoring
and home telehealth,

and telerobotics.

The demand
for rehabilitation services is growing as a result of the aging
population and improved treatment of patients with stroke,
brain injury and other disabling conditions. Because of the
increasing demand, as well as the problems of clinician

shortages and decreased reimbursement, rehabilitation
practitioners are turning to telerehabilitation as a way of
improving access, enhancing the quality of care and
reducing costs.
Human factors in telerehabilitation
It is essential that all telemedicine services are designed and
implemented with the users in mind. While factors such as
age, education and technology experience must be taken
into account when conducting telerehabilitation,
consideration must also be given to patients’ impairment(s).
Deficits in cognitive, gross or fine motor, visual, language or
voice skills all play an important role in patients’ ability to
use the technology and their level of acceptance and
adoption of receiving services at a distance.

Tele-
rehabilitation technology should maximize a patient’s
abilities while minimizing the effect of any abilities that
may have been lost or impaired.

One way to accomplish
this is by applying basic universal design principles, for
example, providing a large button telephone, offering
alternative input methods to devices (such as a touch-screen
or voice recognition), or using simplified large-text or
graphic-based instruction manuals and troubleshooting
guides.
Telerehabilitation programmes must also account for the
range of different clinician users who are involved in all
aspects of service delivery. In addition to the consideration
given to clinicians’ technical knowledge, experience and
training, matters such as professional licensure and
professional scope of practice must also be recognized and
addressed. The American SpeechLanguageHearing
Association and the Canadian Association of Occupational
Therapists are two professional associations which have
tried to educate their members about telerehabilitation and
to develop policies on the delivery of remote rehabilitation
services.

It is paramount that telerehabilitation
clinicians ensure that the services they intend to provide are
within their professional scope of practice and that they
possess the knowledge and skills required to provide them.
Other important human factors issues in
telerehabilitation are the service delivery environment
and the training of users. The design of a telerehabilitation
space must allow patients with mobility aides such as
walkers and wheelchairs to access the facility. In normal
telemedicine practice, it is important to maintain an
uncluttered environment and minimize distractions in the
treatment space.

This is even more important for
telerehabilitation patients who may have decreased levels of
attention (e.g. following traumatic brain injury). In training
telerehabilitation clinicians and support staff, attention
must be paid to ensuring that everyone is familiar with
disability-related matters and is comfortable about
communicating with patients who may have impaired
speech or language. For telerehabilitation patients,
alternative educational techniques and informed consent
procedures (e.g. simplified demonstration rather than
course-based instruction) may be needed to ensure they are
able to fully understand the procedures and the tasks that
will be required of them.
Much as general medicine has seen a gradual shift
towards preventative, proactive and continuous care,

rehabilitation has also become increasingly focused on the
prevention of secondary complications. Rehabilitative care
following a stroke, spinal cord or traumatic brain injury is
often a lifelong process, with recovery occurring at different
rates for each patient. Telerehabilitation for these patients
can be approached using established techniques for disease
management and care coordination.

Home health and
monitoring technologies can be used to help reduce or
avoid dangerous medical consequences (e.g. pressure sores
in patients with spinal cord injuries) and
re-hospitalizations.

While placing technology in the
rehabilitation patient’s home is the most direct way to
improve access to care, it magnifies some of the
considerations already mentioned and raises additional
concerns as user roles become more dynamic and the
service delivery environment becomes less controlled.
In the normal telemedicine delivery model, there are staff
available at each site to assist users during the telemedicine
encounter and help with the set-up and use of any
equipment. However, the home environment is an
unsupervised setting, with on-site assistance only available
from the caregiver. Thus it is essential to provide training for
caregivers who may be asked to act as an impromptu
scheduler, clinician assistant, audiovisual technician or
technology support person. Devices placed in the home
must be simple to operate, reliable and have a high level of
fault tolerance. Remote system diagnostics may be useful for
troubleshooting and maintenance of the equipment at a
distance. Patient privacy must always be protected, so as to
comply with the relevant legal and regulatory requirements,
and also to give patients confidence that they are active
participants in their care, rather than being watched by ‘big
brother’.
Given the broad scope of telerehabilitation, there is no
single generic system that meets the needs of all
applications; rather systems need to be flexible based on
their entire context of use. Figure 2 summarizes and
illustrates several major subsets of human factors
considerations that should be used to guide planning and
delivery of telerehabilitation applications. For example, in
using a computer-based telerehabilitation system to deliver
speech-language treatment to a stroke survivor,

impairments in language, cognition and motor function
necessitate a simple user interface (e.g. with large on-screen
buttons and limited distractions such as toolbars and
menus), an easy way to interact with the system (e.g. a
touchscreen computer monitor) and an environment that
does not distract the patient or detract from the ability of
the clinician to deliver treatment.
Conclusion
Human factors and technology usability play significant
roles in the utilization, acceptance and economic viability
of telemedicine programmes generally.

Thus it is
essential that theories of human factors and user centred
design be applied to the planning and implementation of
new programmes and the design of new devices.

supported by recent telemedicine practice standards and
technical guidelines that incorporate human factors
considerations.

Human factors principles are also central to the growth
of telerehabilitation. With a diverse group of clinicians
and patients, it is essential that all aspects of
telerehabilitation service delivery be accessible, efficient,
understandable and usable by all people regardless of
their age, background or abilities. This will result in
improved access to a wider range of telerehabilitation
services that will facilitate and enhance the rehabilitative
treatment and recovery of people living with varying
levels of injury, impairment and disability.

Background The increasing demand for physical therapy services worldwide calls for innovations to be made to meet the challenges of service delivery. However, little is known about the knowledge, attitude, and practice (KAP) of digital physical therapy among Nigerian physiotherapists. Hence, this study aimed to evaluate the level of KAP of digital physical therapy among physiotherapists in Nigeria. Methods This cross-sectional study purposively recruited 150 Nigerian physiotherapists. Copies of modified and validated questionnaires on the KAP of digital physical therapy from a previous study were sent to registered and licensed physiotherapists via electronic survey to assess the KAP of digital physical therapy. Descriptive and inferential statistics were used to analyze the data. Alpha level was set at p < 0.05. Results The mean age of the respondents was 29.76 ± 6.95 years. Most participants (81.3%) have a knowledge level above average, while 18.7% have below average. Furthermore, most (76.0%) of the respondents had a positive attitude toward digital physical therapy. In addition, more than two-thirds, 69.3% uses digital physical therapy platforms for physical therapy practice. Knowledge, attitude, and practice were not significantly associated with sociodemographic characteristics ( p > 0.05). Furthermore, there was no significant association between knowledge, attitude, and practice ( p > 0.05). Conclusion Many Nigerian physiotherapists demonstrated adequate knowledge, positive attitude, and good practice of digital physical therapy. However, knowledge, attitude, and practice of digital physical therapy were not influenced by sociodemographic characteristics.

Background and Objective. Pre-pandemic, various healthcare settings were not used to seeing patients virtually. The unprecedented need to adopt virtual care during the COVID-19 pandemic may have caught physical therapists (PTs) unready for it. This study aimed to determine the telerehabilitation knowledge, attitude, and practice of PTs in the Philippines during the COVID-19 pandemic and determine the association between demographic and study outcome variables. Methods. This is an analytical cross-sectional study among members of the Philippine Physical Therapy Association, Inc. (PPTA) practicing in the Philippines. Purposive sampling (total enumeration) was employed. All PPTA members were invited to the study through e-mail and official social media group chats. A self-administered questionnaire was used to obtain data on telerehabilitation knowledge (through test questions on various theoretical aspects), attitude, and practice. Results. The questionnaire items had a content validity index of >0.80. The study yielded a 40% response rate. Most respondents were practicing clinicians in urban-based, private rehabilitation centers. Approximately half had average telerehabilitation knowledge, while the majority had agreeable telerehabilitation attitudes across different constructs. Among the respondents, 15.9% used telerehabilitation pre-pandemic, while 64.8% used it during the pandemic. Hybrid (synchronous and asynchronous) telerehabilitation sessions usually lasted one hour per patient, mostly using Facebook Messenger. Conclusion. Telerehabilitation was not widely practiced locally pre-pandemic, which may explain their average telerehabilitation knowledge. The positive telerehabilitation attitudes may represent a small group of PTs favoring telerehabilitation, while information from the larger population remains unknown. Early adopters of telerehabilitation may help introduce virtual care to colleagues and guide them in developing relevant knowledge and skills amid and beyond the enduring COVID-19 crisis.

The digitalization of the world has rendered design for digital inclusion particularly important, which highlights the need to gain a comprehensive understanding of this field. The purpose of this review is to reveal the current development of the field of design for digital inclusion and identify research gaps and directions.Therefore, this study adopted bibliometric mapping to achieve the research goal. A total of 721 relevant articles in English were identified from Scopus. Descriptive analysis, including the publication trend, the most cited journals, the most cited articles, and the top authors with institutions, is described in order to trace the state-of-the-art development of the field. Network analysis, including bibliographic coupling and co-occurrence keywords, was used to identify research themes and future research directions. The results reveal four main investigated topics in the field: (1) information technology; (2) online education; (3) assistive technology; and (4) digital health. The review also highlights the distinctive features of design for digital inclusion compared to inclusive design, discusses the research gaps =, and offers potential future research directions.

Thesis
Ce travail doctoral par article traite des projets de conception de téléréhabilitation (TR) dans les maladies chroniques (MC) et plus particulièrement de la complexité et de la rigueur nécessaire dans la conception de la TR. La contribution majeure de la thèse concerne la proposition d’une nouvelle démarche scientifique actionnable, à partir des connaissances académiques croisées en Systèmes d’Information, en Marketing dans le secteur de la e-santé, permettant aux acteurs de terrain d’agir pour favoriser l’optimisation d’un projet de conception de TR. Concrètement, en identifiant ce qui a été évalué dans la TR puis en intégrant les parties prenantes et l’utilisateur patient, nous cherchions à savoir comment favoriser la réussite d’un projet de conception de TR. Pour ce faire, nous proposons tout d’abord d’avoir une vision d’ensemble des domaines et processus d’évaluation de la TR dans le contexte des MC. Dans le premier manuscrit, nous concluons sur des besoins de multidisciplinarité et d’exhaustivité dans l’évaluation de la TR et confirmons une faible attention portée sur la phase de conception de la TR. A ce titre, nous décidons alors d’approfondir le caractère complexe des projets de conception auquel sont confrontées les différentes parties prenantes. A l’aide d’une étude de cas sur un projet de conception d’un dispositif de téléréhabilitation auquel nous sommes engagés, nous présentons, dans le manuscrit 2 un exemple de défis rencontrés lorsque l’organisation est face à la persistance de paradoxe. Nous montrons que les différents acteurs inter-organisationnels, présents dans le projet de conception, font l’usage d’objets frontières quand le paradoxe, lié à la place du patient comme partie prenante dans la conception, persiste dans le temps. A l’issue d’une analyse approfondie des premiers résultats de cette étude de cas, nous identifions d’autres défis sur les projets dont sont confrontés les différentes parties prenantes. Nous exposons alors dans le manuscrit 3, la manière dont se construit la légitimité organisationnelle d’un projet de conception de TR, qui est une vision originale dans la compréhension d'un projet de conception TI. Nos données montrent notamment que la réparation de la légitimation se situe au niveau symbolique autour d’une partie prenante « dormante », le patient, créant ainsi une cohésion entre les acteurs du projet. Le quatrième manuscrit présente finalement un nouveau terrain de recherche, où le patient n’a pas été partie prenante dans la conception car celle-ci est mise en place dans une situation d’urgence sanitaire. Au sein d’un réseau de santé accompagnant des malades respiratoires chroniques, ce dernier travail de thèse, en cours d’élaboration, vise à présenter la façon dont le patient vit cette expérience de TR en post-implémentation. Notre attention est portée sur le parcours patient intégrant la TR avec un double objectif : d’une part, approfondir la compréhension de ce parcours en restituant l’« expérience » patient et, d’autre part, identifier l’impact sur le processus de conception d’un nouveau service de e-santé comme celui de la TR. La discussion de la thèse aborde les conditions de succès de la démarche scientifique interdisciplinaire, proposée par les travaux de cette thèse, le choix des situations sur laquelle elle s’applique et les perspectives de recherche qui en découlent.

Accepting a robot to perform such measures, e.g., operate in close physical interaction for medical examinations, requires human trust towards the robot. Human-in-the-loop (HIL) applications where the robot is teleoperated by a human expert can help the person to accept even risky tasks performed by a robot. Therefore, the assistive humanoid GARMI was designed to enable HIL applications with varying autonomy. In this study, we use GARMI to understand which tasks in the framework of care may be accepted depending on human socio-demographics and user beliefs as well as the level of robot autonomy. Firstly, we seek to understand the general acceptance of GARMI using the Almere questionnaire. Secondly, we ask adults to rate their willingness to use several functionalities of GARMI. Lastly, we investigate the effect of the introduction method of GARMI on user acceptance. We assemble all relevant factors on acceptance to provide direction in the user-centered design process of assistive robots. The results of 166 participants show that alongside others, trust towards the robot and utilitarian variables such as perceived usefulness are the most influencing factors on the acceptance of GARMI and should be considered for the design of robotic semi-autonomous outpatient-services.

Background and objective: Orofacial myofunctional disorders are common in persons with acquired brain injury. A new way for early detection of orofacial myofunctional disorders via information and communication technologies may improve accessibility. The purpose of this study was to evaluate the level of agreement between face-to-face and tele-assessment of an orofacial myofunctional protocol in a sample of persons with acquired brain injury. Methods: A masked comparative evaluation was conducted in a local association of patients with acquired brain injury. Twenty-three participants (39.1% female, mean age of 54 years) with a diagnosis of acquired brain injury were included in the study. The patients followed a face-to-face and a real-time online assessment using the Orofacial Myofunctional Evaluation with Scores protocol. This is a protocol for evaluation with numerical scales that assess the physical characteristics and the main orofacial functions of patients including appearance, posture, and mobility of lips, tongue, cheeks, and jaws, respiration, mastication, and deglutition. Results: The analysis showed excellent interrater reliability (ρ ≥ 0.85) for all the categories. In addition, most confidence intervals were narrow. Conclusions: This study reveals excellent interrater reliability of an orofacial myofunctional tele-assessment in patients with acquired brain injury in comparison with a traditional face-to-face evaluation.

Introduction Telerehabilitation systems represent a promising way for the management of chronic disability, delivering technology-enabled rehabilitation outside the hospital setting. However, usability and acceptability assessment with users represents a critical starting point when using digital healthcare solutions. This study aims at evaluating the user experience with a Telerehabilitation system (SIDERA∧B) from the end-user side. Methods SIDERA∧B consists of an asynchronous delivery of rehabilitation activities through multimedia digital contents and tele-monitoring of vital parameters with technological devices for individualized, home-based management of chronic conditions. Usability (with the System Usability Scale, SUS) and acceptability (using the Technology Acceptance Model, TAM - and The Service User Technology Acceptance Questionnaire, SUTAQ) data were analyzed from the dataset of the SIDERA∧B project (N = 112 patients with Chronic Heart Failure, Parkinson's Disease and Chronic Obstructive Pulmonary Disease). The possible influence of five external factors (i.e., technological expertise, education, sex, age, and level of disability) on TAM domains was tested using Spearman's Correlation analysis. Results Results showed a satisfactory level of technological usability (SUS Median = 77.5) and good scores in usability and learnability SUS subdomains (mean scores > 2.5). Regarding technological acceptability, participants showed high scores (Median > 4) in “Behavioral Intention”, “Perceived Usefulness”, and “Perceived Ease of Use” TAM domains. Finally, results from the SUTAQ scale highlighted that the SIDERA∧B system obtained optimal scores in all domains, especially in “Increased accessibility,” “Care personnel concerns,” and “Satisfaction.” Age (rho = −0.291, p = 0.002) and disability level (WHODAS Total score: rho = −0.218, p = 0.021) were the two external factors inversely associated with the Perceived Ease of Use. Discussion The age of digital transformation requires everyone to understand, accept and master the changes affecting modern-day healthcare. The usability and acceptability of the SIDERA∧B system were high across all end-users, despite the medium-low level of the technological expertise of the sample. These findings support the efficiency and the suitability of these digital solutions in the modern digital age transition of rehabilitation from inside to outside the clinic.

Zusammenfassung Um den Bedarf an Physiotherapie bei der steigenden Zahl an Patient*innen nach einer endoprothetischen Versorgung auch in Zukunft zu decken, benötigen die Gesundheitsdienstleister*innen eine Lösung für die Bereitstellung einer angemessenen Therapie. Eine mögliche Option könnte die Teletherapie sein, die aufgrund der pandemischen Lage von SARS-COV-2 zunehmend an Bedeutung gewinnt. Teletherapie kann Menschen, die aus diversen Gründen nicht in die Praxis kommen können, eine Therapie ermöglichen, und birgt ein kostensparendes Potenzial. Diese Arbeit sollte klären, ob die physiotherapeutische Teletherapie bei Patient*innen nach einer endprothetischen Versorgung des Knies mit einer konventionellen Physiotherapie vergleichbar ist. Anhand einer Literaturrecherche auf den Datenbanken Cochrane, Europe PMC und PubMed vom 01.01.-04.05.2021 wurden 4 geeignete RCTs gefunden. Alle 4 RCTs zeigten, dass die Teletherapie positive Auswirkungen auf die Funktion der Patient*innen nach einer Knie-Totalendoprothese hat und einer konventionellen Physiotherapie nicht unterlegen ist. Die Ergebnisse waren klinisch signifikant. Die Teletherapie kann demnach als Alternative in Betracht gezogen werden. Allerdings sind manche Studien auf Grund von methodologischen Schwächen kritisch zu betrachten, sodass diese Arbeit keine eindeutige Empfehlung aussprechen kann. Für ein eindeutiges Ergebnis und eine starke klinische Empfehlung bei Patient*innen nach Knie-Totalendoprothese sind weitere Studien nötig.

As the number of persons with disabilities increases because of factors such as aging combined with chronic illnesses, the challenge of providing rehabilitation also increases. Unfortunately, financial considerations have shortened length of stay in acute and subacute settings and reduced rehabilitation services. With earlier discharges, there is an increased need to deliver services to patients in their homes in an efficient and cost-effective manner. Telerehabilitation is an emerging practice that uses communications technology for the remote delivery of rehabilitative care. The Low ADL Monitoring Program is a telerehabilitation program designed to deliver rehabilitation services through the provision of home environmental interventions, together with communications technology and care coordination.

Telemedicine can provide a compelling alternative to conventional acute, chronic and preventive care, and can improve clinical outcomes. In the industrialized world, it is likely that telemedicine will continue to move healthcare delivery from the hospital or clinic into the home. In the developing world or in regions with limited infrastructure, telemedicine will mainly be used in applications that link providers based at health centres, referral hospitals and tertiary centres. The future of telemedicine will depend on: (1) human factors, (2) economics and (3) technology. Behaviours related to technology affect change at the individual, organizational and societal level. Personnel shortages and decreasing third-party reimbursement are significant drivers of technology-enabled health care in the industrialized world, particularly in the areas of home care and self-care. We can safely assume that developments in mobile communications, sensor devices and nanotechnology will alter the way that health care is delivered in the future. The growth and integration of information and communication technologies into health-care delivery holds great potential for patients, providers and payers in health systems of the future. Perhaps the most difficult question to answer, however, is ‘When will telemedicine become part of the standard of care?’

The field of human factors strives to optimise the performance, efficiency, and safety of human-machine systems by obtaining a clear understanding of the cognitive, perceptual and motor capabilities and limitations of users and their environment (both the physical environment and organisational climate). Thus, it is a science expressly devoted to many critical components of telemedicine systems. For example, telemedicine system users interact with potentially unfamiliar technology (e.g. digital cameras), individuals (e.g. doctors and patients), and information (e.g. technical instructions for a referring physician). In addition to these issues that users face on a regular basis, organisational issues exist, such as the management and overseeing of the system and the designation of various user roles within the system. Human factors researchers and practitioners employ multiple methods to evaluate psychological, macroergonomic, and usability issues that face users, as well as to provide design recommendations to improve system efficiency. These methods include task analysis, qualitative research methods (e.g. interviews and surveys), usability analysis, and quantitative research methods (e.g. questionnaires and experimental research). If telemedicine systems are to be successful and widely adopted, human factors evaluations and interventions must play a critical role in their development and continued improvement.

Beginning in April 2000, eight clinical demonstration projects were funded for 2 years within the Sunshine Network of the Veterans Health Administration (VHA) to test disease man- agement principles, the care coordinator role, and the effective use of technology to maintain veterans in their homes. Five of these projects focused on complex medical/chronic disease populations. Seven hundred and ninety-one veterans were recruited in these five projects and enrolled in the Community Care Coordination Service (CCCS). The program was conceptu- alized around and designed by network field staff as an " aging in place" model. The purpose behind the integration of the care coordinator role with technology was to improve health status, increase program efficiency, and decrease resource utilization. Evaluation results to date have shown a 40% reduction in emergency room visits, 63% reduction in hospital ad- missions, 60% reduction in hospital bed days of care, 64% reduction in VHA nursing home admissions, and 88% reduction in nursing home bed days of care. All Performance Improve- ment outcomes reached or exceeded the targeted goals, and a functional assessment revealed five significant improvements out of 10 domains of the SF 36V.

In the last three years, the delivery of rehabilitation services at a distance by means of electronic information and communication technologies has come to be known as telerehabilitation. It is part of both the larger spectrum of telemedicine activities that have waxed, waned and waxed again since the early seventies; and the growing array of non-medical applications of consumer electronics and communication technology that can provide 'tele-enhancement of independent living' for individuals with neurological and other disabilities. While telerehabilitation is still largely hypothetical, activities are underway at several clinical rehabilitation centers. Most commonly, inexpensive video phone connections are used to provide face-to-face two-way image and voice contact between patient and provider, but transmission of data from sensors that monitor health and rehabilitation is technically feasible and promising. Telerehabilitation provides access to quality services for patients who are immobile or geographically remote from direct service. Some view it as a means of compensating for shortened lengths of stay in acute rehabilitation hospitals, while others emphasize the cost reductions it offers. Telerehabilitation methods, both current and in-the-works, are promising but it remains to be seen whether and how the reimbursement policy-makers at HCFA and private payers will respond to its potential.

This article discusses seven core principles that underlie the development of successful telemedicine systems. These principles have been formulated from evidence in the literature and from the personal experience of setting up three telemedicine systems in Australia. Of particular importance has been the observation of failed telemedicine systems, of which there are many examples in the world literature. It is important to learn from history by studying both the successful and the unsuccessful components of these programmes. Telemedicine is still a relatively new area of interest, and while the technology is rapidly evolving, changing and fascinating, it is still the human factors that tend to determine the success or failure of telemedicine projects. These cannot be ignored.

Pressure ulcers are a common and serious secondary complication of spinal cord injury. In addition to being costly and difficult to treat, pressure ulcers may interfere with many aspects of patient and family life, including the ability to meet educational, vocational, and social goals. Treatment of pressure ulcers includes weekly assessment by a clinician, a requirement that often is impossible for clients to meet. In an effort to improve outcomes in wound care treatment, a rehabilitation center undertook an exploratory project to determine whether wound care via telerehabilitation was a viable alternative to clinic visits. Telerehabilitation is the use of telecommunication technology to deliver rehabilitation services at a distance. Eight patients being followed in the outpatient clinic participated in the project. The Picasso Still-Image Videophone was used to capture and send images from the patients' homes to the clinic. Findings from the exploratory study demonstrated that pressure ulcers can be successfully managed via telerehabilitation.

Human factors engineering and system design are critical elements in the newly developing field of telerehabilitation. Telerehabilitation is the remote delivery of rehabilitative services such as monitoring, training, and long-term care of persons with disabilities using telecommunications technology. This paper describes projects at the Rehabilitation Engineering Research Center (RERC) on Telerehabilitation in the context of three conceptual models: telecounseling and training, telemonitoring and assessment, and teletherapy. Issues pertaining to human factors engineering design are identified, and ongoing challenges are discussed.
