ORAL SQUAMOUS PAPILLOMA OF BUCCAL MUCOSA: A CASE REPORT

Abstract

Oral squamous papilloma is a benign proliferation of the stratified squamous epithelium, which results in a papillary or verrucous exophytic mass, that arise from the mucosal surface. The foremost common site is the palate uvula area followed by tongue, lips and buccal mucosa. The etiology remains unknown. Treatment usually requires total excision and recurrence is rare. Here, we are reporting a case of squamous papilloma in the left side of the buccal mucosa in 50 year old male patient. Keywords: squamous papilloma, buccal mucosa, tumor, excisional biopsy

Introduction
Oral
squamous papilloma
is
a common
lesion
of
the oral mucosa. It's a benign
proliferation
of
the stratified squamous
epithelium, which ends up
in
a well
demarcated, white
or
normal colored lesion,
that arise from the mucosa! surface. The
sites
of
predilection for localization
of
the
lesions include the tongue, lips and soft
palate (20%), but any surface
of
the mouth
can
be
affected. The intraoral papilloma
typically
is
clinically presented
as
slow
growing, well-demarcated lesion, usually
with normal coloured mucosa and smooth
surface, sessile or pedunculated base, and
hard consistency,causing difficulties
in
mastication and speech. The dimensions
of
the growth
is
generally smaller than
1.5
cm.We presenta case
of
0,9 cm
of
squamous papilloma arising from buccal
mucosa
in
oral cavity.
Case presentation
A
50
years old
man
was referred
to
the
University Dental Clinical Center "St.
Pantelejmon", Skopje, North Macedonia.
Thus, the patient reported a presence
of
a
"ball"
in
his mouth approximately three
months. Hecomplained on hurting the cheek
while eating. This painless ball became
ulcerous, started bleeding and accompanied
with pain. Nevertheless, the entire medical
history revealed
no
systemic diseases. The
left submandibular lymph nodes were non
palpable and non tender. The intraoral
examination revealed a soft nodule with size
1,0
x 0,5cm, located on the buccal mucosa
of
the left side, covered by normal and
healthy soft tissue. Intraorally on palpation,
it
was ovoid and firm
in
terms
of
consistency (Fig. I).

Oral squamous papilloma is a benign tumor whose pathogenesis has been associated with human papillomavirus infection. Thus, it is noteworthy that human papillomavirus infection is one of the risk factors associated with the development of cervical, anogenital, pharynx, larynx and oral cavity carcinomas. Oral squamous papilloma can affect any region of the oral cavity, and transmission of human papillomavirus can occur by direct contact, sexual intercourse or from mother to child during delivery. The diagnosis is clinical and histopathological, with surgical removal representing the treatment of choice. Recently, widefield optical fluorescence has been used as a complementary examination to the conventional clinical examination in the screening of oral pathological lesions and for the delimitation of surgical margins. We report a case of oral squamous papilloma with its clinical, histopathological features and, in addition, from the perspective of wide field optical fluorescence.

Squamous papillomas are common lesions of the oral mucosa with a predilection for the mucosa of the hard and soft palate. As an oral lesion, it raises concern because of its clinical appearance, which may mimic exophytic carcinoma, verrucous carcinoma or condyloma acuminatum. Its pathogenesis is related to human papilloma virus but there is controversy regarding its viral origin. We present a case of squamous papilloma presenting as oral lesion along with a review of the literature.

Genital infection by human papillomavirus (HPV), a sexually transmitted disease (STD), has increased considerably due to the changes in sexual behaviour and an increase in the practice of oral sex. HPV, in a parallel manner, has been closely studied due to its oncogenic potential. We present the case of a 27-year-old patient, with a multi-partner sexual history and frequent practice of oral sex, who suffered from warts lesions on the genitalia and tongue. Squamous papilloma was diagnosed from a tongue biopsy. The treatment of the oral lesion was by way of surgery, without relapse in the first two years. Our discussion in this report is regarding the HPV infection in the oral cavity.

Public awareness of human papillomavirus (HPV) as the causal agent of cervical cancer and of the availability of HPV vaccines has increased. As a result, more patients are asking their dentists about oral HPV infection and its prevention by means of vaccination. Parents of pediatric dental patients also may be concerned when their children have HPV-associated oral lesions, because HPV infection still often is considered a purely sexually transmitted disease. In this review, the authors provide the latest information for dental professionals about HPV infection in the oral mucosa and in general. The authors searched PubMed for all studies regarding HPV infection in the oral mucosa, and they reviewed relevant publications focusing exclusively on HPV infections of the oral cavity. In selecting studies for review, the authors made a clear distinction between studies regarding HPV infections in the mouth and those regarding HPV infection in the oropharynx or in other head and neck sites. HPV can infect oral mucosa. A subgroup of oral cancer clearly is associated with HPV. Oral HPV infection is transmitted sexually but also can be transmitted from mouth to mouth and vertically from an infected mother during delivery. Persistent HPV infection in the oral mucosa might increase the risk of developing oral cancer. Regular and meticulous clinical examination is the dentist's most important tool in detecting HPV-associated changes in the oral mucosae. HPV-associated oral cancer may affect a population younger than that typically affected by HPV-independent oral cancer. Alcohol and tobacco use increase the risk of developing oral cancer, so good practice includes encouraging patients to avoid these habits. The available HPV vaccines cover the HPV genotypes found most commonly in the oral mucosa, but their protective effect against oral cancer remains to be elucidated.
