We report a case diagnosed as both necrotizing fasciitis and frostbite of the male external genetalia. The patient was a 58-year-old man with diabetes mellitus. He was referred to our hospital with swelling of his penis and icing there for 3 weeks. We diagnosed his condition as necrotizing fasciitis of the external genitalia and decided to perform an emergency operation. Although we consulted plastic surgeons about debridement of a large area, they considered that there was no necessity for it because the scrotum, perineum and abdominal wall were frostbitten. The final clinical diagnosis was necrotizing fasciitis of the penile region and frostbite around the perineal region. We performed partial penectomy without debridement. Through collaboration with another medical department it was thus possible to avoid unnecessary invasive treatment.

Portals of entry were perirectal abscesses in five patients, urethral infections in three, and surgical procedures in four patients. All patients required aggressive surgical debridement, broad-spectrum antibiotics, and adjunctive measures. The fact that four patients died in spite of aggressive treatment demonstrates the lethal nature of this disease. This study suggests that this syndrome is no longer "idiopathic" but is primarily a necrotizing cellulitis of the perineum with subsequent involvement of the genitalia and surrounding tissues.(Arch Surg 1983;118:38-40)

[Show full abstract] patients with Fournier's gangrene between the years 1985 and 2003 who, after initial treatment by the Departments of Urology and Surgery, were referred to the Department of Plastic Surgery for reconstruction. The following parameters were evaluated: age, gender, interval between onset of symptoms and diagnosis, clinical symptoms, lesion site, results of bacteriologic cultures, cause and predisposing factors, treatment and reconstructive procedures, length of hospital stay, and outcome. The mean patient age was 56.6 years. Fifteen patients (34.9 percent) had diabetes mellitus. The cause of Fournier's gangrene was found in 32 patients (74.4 percent). The most common presentation was scrotal swelling, and scrotal involvement was found in 40 cases (93.0 percent). All of the patients underwent surgical debridement, and several reconstruction techniques were used. The mean length of hospital stay was 73.6 days. Two patients died. Management of this infectious entity should be aggressive. Several techniques that are used to reconstruct the lost tissue have shown good results. The superomedial thigh skin flap has proven to be a reliable method of resurfacing large scrotal defects. Reconstructive surgery makes the return to a normal social life possible in many cases.

Clinical case: a sixty-two-year-old male patient of mixed race that comes to the emergency service with necrotizing fasciitis of scrotum. He is admitted in the Urology Service of the Celia Sanchez Manduley University Hospital and undergoes an analgesic, anti-inflammatory treatment and antibiotic therapy that evolved to the necrosis of the scrotum. It was noticed that the patient was eager and worried about the appearance of his genitals. It was seen that the skin of the scrotum that covered the right testicle was almost intact. The skin of this area has the characteristic of being very elastic; the dissection of the skin was made until there was enough tissue to cover the left testicle. Conclusions: functional and aesthetic results were achieved. Thanks to the applied technique, the patient recovered with a minimum of aesthetics sequelae.

Patient's age, source and predisposing factors, microbiological findings, duration of hospital stay, treatment modalities, and outcome were analyzed. The mean age of the patients was 53.9 +/- 9.56 (range 23-71) years. The source of gangrene was urinary in 5 patients, perirectal in 5, cutaneous in 1, and unknown in 9 patients. The main predisposing factors included diabetes mellitus in 9 patients (45%) and immunosuppression in 5 patients (25%). The mean duration of hospital stay was 39 +/- 10 (range 6-62) days. Although early intervention and intensive treatment were carried out, 4 patients died with an overall mortality of 20% as a result of septicemia complications. The mortality rate was higher in elderly patients and those with diabetes mellitus, but it was not statistically significant. Regarding the mortality rate, duration of symptoms, number of debridements, culture results and source of infection were not found to be significant factors. FG is still a severe disease. Management of this infectious entity must be aggressive. Despite the use of contemporary effective antibiotic treatment, aggressive debridements, and state-of-the-art intensive-care conditions, FG still has high mortality and morbidity rates.

Isolated Penile Fournier's gangrene: A case report and literature review

The patient was treated with immediate surgical debridement after he received broad-spectrum antibiotics. When the wound exhibited healthy bed, we used an unexpanded, meshed, split-thickness skin graft for penile reconstruction. The recovery phase was uneventful with satisfactory cosmetic results. Discussion: FG is a rare necrotizing infection of the perineum and genital region with a high mortality rate that can reach up to 50% of cases. It is frequently due to polymicrobial infection. The diagnosis is often made clinically, although radiologic studies can be helpful to define the extent of the disease. The clinical features of FG include sudden pain and swelling in the scrotum or wound discharge. Isolated penile FG are documented in few cases where the corpora cavernosa are usually spared. Its presence are related to patients sexual habits or traumatic insult. Mainstays of treatment include rapid and aggressive surgical debridement of necrotized tissue, broad-spectrum antibiotic and early resuscitation. Conclusion: Only a few cases of penile FG have been reported. Early debridement of all necrotic tissue is the golden rule.

The first 21 patients (Group I) were treated with broad-spectrum triple antimicrobial therapy, broad debridement, exhaustive cleaning, and then they underwent split-thickness skin grafts or delayed closure as needed. The other 12 patients (Group II) were treated with unprocessed honey (20-50 mL daily) and broad-spectrum triple antimicrobial therapy without debridement. Their wounds were cleaned with saline and then dressed with topical unprocessed honey. The wounds were inspected daily and the honey was reapplied after cleaning with normal saline. Then, the patients' scrotum and penis were covered with their own new scrotal skin. The mean age of the patients was 53.9 +/- 9.56 years (range = 23-71). The source of the gangrene was urinary in 23 patients, cutaneous in seven patients, and perirectal in three patients. The predisposing factors included diabetes mellitus for 11 patients, alcoholism for 10 patients, malnutrition for nine patients, and medical immunosuppression (chemotherapy, steroids, malignancy) for three patients. The mean duration of hospital stay was 41 +/- 10.459 (range = 14-54) days. Two patients in Group I died from severe sepsis. The clinical and cosmetic results were better in Group II than Group I. Necrotizing fasciitis of the perineum and genitalia is a severe condition with a high morbidity and mortality. Traditionally, good management is based on aggressive debridement, broad-spectrum antibiotics, and intensive supportive care but unprocessed honey might revolutionize the treatment of this dreadful disease by reducing its cost, morbidity, and mortality.

Management of Fournier's Gangrene: Review of 45 Cases

The mortality rate was higher in patients with diabetes mellitus, but it was not statistically different. The age, duration of the symptoms, and the presence of rectal abscess were not found to be significant factors regarding mortality rate. Surgery with extensive debridement of all necrotic tissue is the main stay of treatment.
