Abstract

Dermoid cyst is an uncommon developmental anomaly usually congenital or hereditary in nature which can result from either defective genetics or from a genetic insult/agent that's associated with the fetal environment or from their interaction. Dermoid cysts and congenital diseases are not most common in Bangladesh and the economic effect of congenital defects is not as high as compare with that of other diseases that have high economic effects. The main objective of this study is to report severe bilateral corneal dermoid with nasal hyperplastic tissue in a calf and its therapeutic measurements attended in veterinary teaching hospital, BAU.

INTRODUCTION
Congenital means present at birth and is not synonymous with hereditary (Samad, 2008). Ocular dermoid is a
skin or skin-like appendage usually arising on the limbus, conjunctivae, and cornea that may be unilateral or
bilateral and associated with other ocular manifestation or with other malformations. Hair from the lesions is
predominantly responsible for the associated irritation resulting in chronic inflammation of the conjunctivae and
cornea and may cause visual impairment (Barkyoumb and Leipold, 1984; Greene et al., 1973; Moore et al.,
1999). Dermoids may affect the eyelids, conjunctiva, nictitans, sclera and cornea (Lawson, 1975) and most
commonly present unilaterally. Bilateral ocular dermoids have been reported in cattle (Barkyoumb and Leipold,
1984; Yeruham et al., 2002) and in camel (Moore et al., 1999). Ocular dermoids are rare in cattle, with the
prevalence estimated between 0.002% and 0.4% (Yeruham et al., 2002). A study on bovine congenital defects
was carried out in Bangladesh and ocular system was recorded including dermoid cyst (7.14%) (Hasan et al.,
2015; Samad, 2002). This paper reports a case of bilateral large dermoid cyst and nasal hyperplastic tissue in a
calf.
CASE HISTORY
A two days old calf had been admitted at veterinary teaching hospital, Faculty of veterinary science,
Bangladesh Agricultural University, Mymensingh with a complain of difficulty vision with extra growth of
conjunctiva with an abnormal appearance of both eyes and difficulty in breathing with a sign of open mouth
breathing since birth. A large fleshy mass was attached to the cornea and sclera with a large number of
temporally and superiorly directed hairs arising from the surface of the mass resulting blindness and continuous
lacrimation. An overgrowth in nostril resulting difficulty in breathing and open mouth breathing. The clinical
diagnosis was bilateral corneal dermoids and nasal hyperplastic tissue.
The calf was admitted to the veterinary teaching hospital after initial examination. The calf weighed 25kg, was
in good bodily condition and no other abnormalities were detected on physical examination.

TREATMENT
After clinical examination, calf was prepared for surgical excision of dermoid cysts. As the calf was 2 days old
the operation was performed in two successive attempts of 7 days apart instead of both eyes at a time. The
operation site was prepared for aseptic surgery by shaving, scrubbing and application of mild antiseptic solution.
The calf was sedated with Diazepam 10mg (Sedil®, Square Pharmaceuticals,Dhaka, Bangladesh) 2 mL ampoule,
IM and restrained on table in lateral recumbency. Auriculopalpebral and Peterson nerve was blocked by local
infiltration with 2% lignocaine hydrochloride (Jasocaine®, Jayson Pharmaceuticals, Dhaka, Bangladesh) 5 mL.
The cysts were excised by superficial lamellar keratectomy that was extended into the third eyelid, medial
canthus, eyelid and bulbar conjunctiva. Cysts were carefully dissected from all adherent areas, stalks were ligated
with chromic catgut (No. 1). The nasal masses were excised with first eye operation.

POSTOPERATIVE TREATMENT AND FOLLOW UP
Systemic amoxicillin 100mg/mL (Moxacil vet®) 2 mL 48 hours interval were administered IM for five days in
addition to a local antibiotic and anti-inflammatory (for eye instillation). The calf recovered uneventfully. The
calf was again examined after one month for rule out any reoccurrence. Re-examination after one month did not
reveal any re-occurrence of dermoid cysts.

The common congenital surgical problems in calves includes atresia ani, atresia ani et recti, atresia ani et recti et coli, atresia ani with rectovaginal fistula, umbilical hernia, contracted tendon, eventration of intestine, pervious urachus, meningocele, patellar luxation, lip and palate defects, maxillo-mandibular and ocular defects. Surgical treatment of such calves is of utmost significance for their survival. The present review deals with common congenital surgical problems in bovine and their surgical management.

Background: Congenital malformations are structural or functional anomalies that occur during intrauterine life and can be identified prenatally, at birth or sometimes only later during life. These congenital defects can evolve from abnormal genetic or environmental factors or a combination of both but the causes of many cases are unknown. The risk factors are associated with congenital defects varies depending on species, breed, body system, geographical location and season. Some congenital malformations can cause death of newborn, while others can only create a structural defect without affecting vital functions and responsible for slowing down of genetic progress and economic loss for the breeders. Congenital defects constitute the at least fifth largest cause of neonatal morbidity and mortality but national estimate of the prevalence of these anomalies are lacking in inland literature. Thus, it is important to estimate the prevalence with identify the causes and risk factors of congenital defects for their possible prevention to minimize the economic losses in animal industry. Objective: The objective of the review was to derive an estimate of the birth prevalence and associated host risk factors of congenital anomalies reported in calves and kids in Bangladesh. Materials and Methods: The occurrence of congenital anomalies in new born farm animals has been reviewed from the first report published in 1975 up to 2021 in Bangladesh. The search was carried out in Google, PubMed, Banglajol and also the original inland journals. Results: All types of search identified 33 inland articles and 35 articles published from foreign nations. Of the 33 available inland articles of which most of the articles based on either single clinical report or data abstracted from veterinary hospital records. Approximately 36 types of congenital malformations were recorded, of which 31 types in cow calves and only five types in kids. Higher occurrence of congenital defects reported in calves (96.95%; n = 1746 cases) than kids (3.05%; 55 cases), but both the calves (56.25%; n = 923) and kids (67.27%; n = 37 cases) affected with atresia ani comparatively higher than other reported congenital malformations. System-wise analysis on prevalence of congenital anomalies found highest prevalence in digestive system (67.18%), followed by ocular system (17.07%), musculoskeletal system (8.42%), urogenital system (2.58%), integument system (2.52%) and others. Comparatively higher prevalence of congenital defects reported in male (65.57%) than female (34.43%) and crossbred (67.03%) than indigenous (32.97%) calves in Bangladesh. Conclusion: Calves and kids born with various congenital defects in their different body systems with highest occurrence of atresia ani in both calves and kids have been recorded. Calves have a great variety of defects than kids probably due to genetic causes. Atresia ani and ocular dermoids are the most frequent occurrence but several cases are still not reported due to lack of disease monitoring system, leading to an underestimation of the real weight of congenital malformations especially in cross-bred cattle. Although some congenital defects can be corrected surgically, the majority cases may not be easily corrected surgically moreover surgically corrected animals may not be used for breeding purposes. Therefore, prevention of the occurrence of congenital defects is required based on the identification of causal factors.

Four different forms of ocular dermoid in dairy cattle herds are reported. The incidence is estimated by 0.002%. This congenital anomaly seems not to be inherited, but it may appear with other congenital eye defects or with other malformations.

Nature and cause of bilateral ocular dermoids were investigated by field studies, pedigree analysis, clinical examination, light microscopy, scanning electron microscopy, and transmission electron microscopy. It was determined that ocular dermoids in Hereford cattle are a genetically transmitted defect; characteristics of autosomal recessive and polygenic inheritance were observed. Calves typically were affected bilaterally with multiple, connected ocular growths that clinically, histologically, and ultrastructurally mimicked normal haired skin. Sites most commonly involved included ventro-lateral limbus, third eyelid, medial canthus, eyelid and conjunctiva. Centro-corneal and anterior segmental dermoids also were observed.

A 5-wk-old female dromedary camel (Camelus dromedarius) was clinically diagnosed with bilateral corneal dermoids, incomplete congenital cataracts, a left persistent hyaloid artery (PHA), and a ventricular septal defect (VSD). The corneal dermoids were removed by lamellar keratectomy, and vision improved in the left eye. Thirteen months after dermoid surgery, the calf was presented for enlargement of the right eye. Glaucoma was confirmed in the right eye, and corneal fibrosis and cataract were noted in the left eye. Persistence of the VSD was confirmed by cardiac ultrasonography. The calf was euthanized, and necropsy findings confirmed VSD. Histopathologic examination revealed bilateral corneal thinning and fibrosis, cataracts with retrolental fibroplasia, and retinal dysplasia. Additional changes in the right globe were anterior segment dysgenesis, ruptured lens capsule, chronic phacoclastic uveitis, and retinal separation. The PHA was confirmed in the left eye.
