Lack of Elevated Serum Carcinoembryonic Antigen and Calcitonin in Medullary Thyroid Carcinoma

Medullary thyroid carcinoma (MTC) originates from C-cells. A wide variety of tumor markers including calcitonin (CT), carcinoembryonic antigen (CEA), and chromogranin A are produced by MTC. The following case concerns a ... [Show full abstract] 50-year-old woman with histologically proven MTC, who completely lacked serum elevation of both CT and CEA. We performed a total thyroidectomy with lymphadenectomy in the central compartment. Histologic sections were stained for CT, CEA, and chromogranin A. Additionally we examined the patient's blood for mutations in the RET proto-oncogene. Serum CT and CEA were below the detection level in the serum. The tumor showed weak staining for CT, but strong staining for CEA and chromogranin A. Sequencing of the RET-proto-oncogene revealed no mutations. Five years after the operation, the patient remains well and shows no signs of tumor recurrence. We hereby report of a patient with neither plasma elevation of CT nor CEA. From the clinical standpoint, it is important to determine how this subgroup of MTC should be followed because CT and CEA are of no clinical use.

[Show full abstract] level of carcinoembryonic antigen (CEA) (54.4 ng/ml; normal limit <5.0 ng/ml) was significantly elevated. Ultrasonography identified a hypoechoic lesion. Mammography revealed a hyperdense lesion with a well-circumscribed margin. The patient underwent a modified radical mastectomy with axillary lymph node dissection. Pathology showed tumor cells with neuroendocrine features, with diffuse immunopositivity for chromogranin and synaptophysin. The tumor cells were also strongly positive for progesterone and estrogen receptor, but negative for HER-2/neu expression. The CEA value gradually decreased to the normal range within one month after surgery. Neither recurrence nor distant metastasis has been detected at 20 months after surgery and hormone therapy with letrozole. The serial CEA levels were within normal limits in the follow-up period. The serum CEA level after surgery may be a potential marker for evaluating tumor recurrence or prognosis of patients with PNCB.

In the management of the patients it is also important to have tumor markers that can give prognostic information. Recent findings might indicate that CA-50 could be an indicator of malignant disease and possibly also correlate with survival.
