Let not thy food be confused with thy medicine: The Hippocratic misquotation

Abstract

Background and aim: The link between food and health has been documented since Antiquity. The aim of the article is to show that the alleged Hippocratic phrase “let food be thy medicine and medicine be thy food” is a widespread misquotation, lying at the root of an entire misconception about the ancient concepts of food and medicine. Methods: Examples of publications gathered from the MEDLINE� and Google Scholar databases which cite the phrase verbatim and attribute it to Hippocrates were selected. Then, an extensive review of the works related to food and diet in the Corpus Hippocraticum was made in order to search for the phrase. Results: At least for the last 30 years this phrase has mistakenly helped scientists confirm the importance of food to health and highlight new ethical challenges in medicine and dietetics. We showed that at least one biomedical journal per year has cited the phrase. Conclusion: This literary creation has led to an essential misconception. For Hippocrates, even if food was closely linked to health and disease, the concept of food was not confused with that of medication.

1. Introduction
The importance of food in medicine was recognized in the 5th
Century BC by Hippocrates of Cos, who is considered the father of
Western medicine. His work was compiled either directly or indi-
rectly through his disciples, so that the existing knowledge on
Hippocratesmedicine consists of more than 60 texts known as The
Hippocratic Corpus (Corpus Hippocraticum). This important text in
the history of medicine expounds on the theory of diet.

However,
the phrase let food be thy medicine and medicine be thy food,a
widespread phrase quoted by todays scientists, is nowhere to be
seen. This literary creation is not only a misquotation but it also
leads to an essential misconception: in the Hippocratic medicine,
even if food was closely linked to health and disease, the concept of
food was not confused with that of medicine.
2. Medicine and diet in the Hippocratic era
Hippocratic medicine was considered as an art and a science
based on two dietary imperatives. First, the necessity to adapt
healthy peoples food to human nature: this implied cooking it,
therefore differentiating it from animals. The second imperative
was to modify and adapt the diet of sick patients according to their
condition, in order to avoid suffering and death. In the Classical
theory of humoral pathology, based on the concepts of natural
philosophy of that time, health and disease states were related to
the four bodily uids or humors: blood, phlegm, yellow bile and
black bile. Health was related to the balance of the four humors in
terms of quantity and composition. Disease appeared in case of
humor imbalance. Elements like food and seasons were then
identied as possible causes. Up until Hippocrates, diseases had
been seen as a consequence of divine intervention. With him, they
became seen as a state caused by natural causes, including diet.

In order to ght diseases, Hippocratic doctors used two kinds of
interventions. On the one hand, the previously existing therapeutic
interventions such as medicines, incisions, and cauterization and
on the other hand the new regimen or dietetic interventions. In a
hierarchical order, the most important intervention was diet. Sec-
ondly, medicines seemed to be considered as means of evacuation
or purgation of impure uids from the various cavities of the body.
Incisions with iron utensils to bleed the patient had the same
objective as medicines. If any of these last interventions failed,
burning cauterizations were the last option. The dietetic interven-
tion, which included a food regimen and exercises, was considered

revolutionary at the time. The properties of foods were meticu-
lously analyzed in the treatise On Regimen. Physicians were then
able to prescribe a detailed food regimen to patients based on their
individual nature, activity, age, season, etc.

Thus it is considered
that medicine in the Hippocratic era was in fact mainly a dietetic
medicine, not a pharmacological or surgical medicine.
3. Food and medicine misconception
But Hippocratic doctors clearly saw a difference between food
and medicines. In fact, food was considered as a material that could
be assimilated after digestion (e.g. the air was also food) and con-
verted into the substance of the body. For example, food was con-
verted into the different parts of the body such as muscles, nerves,
etc. By contrast, the concept of medicines at the time was a product
which was able to change the bodys own nature (in terms of humor
quality or quantity) but not be converted into the bodysown
substance. Thus a food wasnt considered a medicine. A possible
root of the food-medicine confusion is the following cryptic phrase
found in the work On Aliment:In food excellent medication, in
food bad medication, bad and good relatively.

This text is nowa-
days attributed to the Hellenistic period, but was considered to be
Hippocratic in Antiquity by Galenus in particular.

Finally, the guidelines pertaining to the duty of physicians in
Antiquity were established in the Hippocratic Oath whose legacy
still persists. The imperative Do no harmis implicit in the
following statement concerning diet: I will apply dietetic measures
for the benet of the sick according to my ability and judgment; I will
keep them from harm and injustice.

Thus diet became with Hip-
pocratesendorsement a moral value and responsibility in the area
of medical practice.
4. The Hippocratic mythical quotation
There is no doubt about the relevance of food in The Hippocratic
Corpus and its role in health and disease states. In fact, in order to
conrm and emphasize the importance of food to health and to
highlight new challenges to medicine and dietetics, scientists have
been citing this phrase at least for the past 30 years.

shows some examples of publications gathered from the
MEDLINE

and Google Scholar databases which cited the phrase
verbatim. All these publications dealt with nutrition and health
concerns, and were aimed at proving the role of nutrition in
different elds such as cancer, epigenetics, immunology, disease
prevention or chronic diseases. Some publications aimed to vali-
date and legitimate scientically and ethically the current concepts
of nutraceuticals or functional foods from Antiquity.

However,
by attributing pharmacological properties to foods, authors are
confusing both food and medicine. In fact, none of the authors cited
the original text of this alleged Hippocratic phrase accurately. Two
of the examples referred to no primary sources.
References and quotation errors are relatively common in
biomedical journals and are not a new concern.

This problem is
found in all types of journals and the rate of errors is independent of
the journals impact factor, showing that journal quality does not
necessarily correlate with reference accuracy, as has been shown
previously for surgery journals.

The primary responsibility for
accuracy of reference and citation lies on the author. However, this
aspect is frequently neglected, all the more as the policy in most
journals is not to correct an error in reference. Several consequences
resulting from misquotation could be considered. First, the original
author may be displeased to see their original concepts or state-
ments misunderstood. Second, the reader may be misled; and last,
the most serious consequence of inaccuracy and misinterpretations

in reference or quotation, is that mistaken information may become
a truthor an accepted fact. This is the case of the Hippocratic
misquoted phrase. Todays scientists have accepted it improperly as
true that the concepts of food and medicine were confused in An-
tiquity by Hippocrates.
5. Conclusion
Misquotation can lead to persistent misconceptions. Let food
be thy medicineis a fabrication that was accepted worldwide
based on the iconizing of Hippocrates, who appears to give moral
and ethical sanction to the phrase. Yet, though food and medicine
have been highly related since Antiquity, for Hippocrates, they were
not conated as scientists claim today. More research is warranted
to ascertain the precise origin of the Hippocratic misquotation.

Introduction Over the last two decades research has grown regarding dairy intake and health. It has been reported by many that yogurt intake may be associated with reduced risk of type 2 diabetes mellitus (T2D). In this report, the United States Food and Drug Administration (FDA) decision to announce a qualified health claim for yogurt products regarding reduced risk of T2D in response to a Danone North America petition is discussed. Methods Relevant literature cited in the petition along with supporting evidence from PubMed and Google Scholar databases until April 1st, 2024 were used. Literature was found using relevant keywords. Results On March 1st, 2024, the United States Food and Drug Administration (FDA) announced the first ever qualified health claim, stating that there that eating yogurt regularly may reduce the risk of T2D according to limited scientific evidence. The enforcement discretion letter was critically reviewed and discussed regarding its future implications for people with T2M and public health. Conclusions It is unclear how this FDA decision will affect public health and nutrition in the long-term. Limited scientific evidence suggests that at least 3 servings of yogurt per week may reduce the risk of T2D incidence for the general population. Yogurt will not cure or treat people with T2D.

Recognised since antiquity, gout is still a relevant pathology with rising prevalence and incidence. This study aims to assess the reference accuracy in journal articles mentioning the early use of the word ‘gout’. Specifically, it investigates whether the term was indeed coined in the 13th century by the Dominican monk Randolphus of Bocking, as widely believed. Several historical sources in the original Latin were consulted to test the hypothesis of literary mentions predating Randolphus of Bocking’s description. At the same time, biomedical articles spanning the last two decades were perused using specific keywords in different combinations to determine the accuracy level of references related to the earliest use of the word ‘gout’. The results showed that several biomedical publications wrongly ascribed the origin of the word ‘gout’ to Randolphus of Bocking. Indeed, various texts predate Randolphus of Bocking’s mention by many years. In particular, gutta, the Latin word used to indicate a host of rheumatological conditions including gout, is recorded as early as the 10th century in a biography dedicated to the martyred nun Saint Wiborada of St. Gall. Written by Swiss monks between AD 960-963, this text should be regarded as containing the earliest known adoption of the word. For this reason, scholars should now avoid quoting Randolph of Bocking’s description as the first use of the word gout in Western literature.

The phrase “Let food be thy medicine…” means that food can be a form of medicine and medicine can be a form of food; in other words, that the diet we eat can have a significant impact on our health and well-being. Today, this phrase is gaining prominence as more and more scientific evi-dence suggests that one’s diet can help prevent and treat disease. A diet rich in fruits, vegetables, whole grains, and lean protein can help reduce the risk of heart disease, cancer, diabetes, and other health problems and, on the other hand, a diet rich in processed foods, added sugars, and satu-rated fats can increase the risk of the same diseases. Electrophilic compounds in the diet can have a significant impact on our health, and they are molecules that covalently modify cysteine residues present in the thiol-rich Keap1 protein. These compounds bind to Keap1 and activate NRF2, which promotes its translocation to the nucleus and its binding to DNA in the ARE region, triggering the antioxidant response and protecting against oxidative stress. These compounds include poly-phenols and flavonoids that are nucleophilic but are converted to electrophilic quinones by met-abolic enzymes such as polyphenol oxidases (PPOs) and sulfur compounds present in foods such as the Brassica genus (broccoli, cauliflower, cabbage, Brussel sprouts, etc.) and garlic. Algae and their food products are in a great interest worldwide because of their nutritional value and health benefits. These edible substances are an excellent supply of antioxidants, high-quality protein, and fibers. The present work therefore targeted assessing Arthrospira platensis (A. platensis) (Cyanobacteria) and Chlorella vulgaris (C. vulgaris) (Chlorophyta) nutritional value and exploring A. platensis and C. vulgaris effects on hepatic enzymes and antioxidants in diabetic rats. A total of 48 healthy adult albino male rats of the Sprague-Dawley strain were assigned into 8 groups (n = 6). They were gavaged with 0, 100, 200, and 300 mg/kg of Ap and Cv extract. The first group were healthy rats fed on basal diet only, also known as the control negative (−ve) group, and seven other groups which induced diabetes by 120 mg/kg alloxan were as follows: groups 3, 4, and 5 treated with Spirulina are given 100, 200, 300 mg/kg of body weight, respectively, for 30 days and groups 6, 7, and 8 treated with Chlorella are given 100, 200, 300 mg/kg of body weight, respectively, for 30 days. By the end, sampling of blood was done to evaluate some biochemical markers encompassing trace minerals, hepatorenal functional indexes, malondialdehyde (MDA), antioxidants, and total protein (TP). Current findings revealed significant increase of plasma concentration hepatic enzymes (AST, ALT), creatinine, urea, and MDA but decrease in antioxidant enzymes, GSH, and TP in diabetic rats. However, an oral supplement of A. platensis or C. vulgaris (100, 200 ,and 300 mg/kg of body weight) lowered the levels of AST, ALT, creatinine, urea, and MDA and increased antioxidants and Tp. Diabetes decreased the plasma levels of antioxidants and also increased hepatic enzymes. The inclusion of A. platensis and C. vulgaris supplementing 100, 200, and 300 mg/kg of body weight increased some antioxidant enzymes, Tp, and GLP while lowered the levels of MDA, AST, ALT, creatinine, and urea. Our findings suggest that A. platensis and C. vulgaris are promising natural sources with nutritional and functional properties that allowed them to be used for the nutritional management of diabetic patients.

Personalised nutrition (PN) has emerged over the past twenty years as a promising area of research in the postgenomic era and has been popularized as the new big thing out of molecular biology. Advocates of PN claim that previous approaches to nutrition sought general and universal guidance that applied to all people. In contrast, they contend that PN operates with the principle that “one size does not fit all” when it comes to dietary guidance. While the molecular mechanisms studied within PN are new, the notion of a personal dietary regime guided by medical advice has a much longer history that can be traced back to Galen’s “On Food and Diet” or Ibn Sina’s (westernized as Avicenna) “Canon of Medicine”. Yet this history is either wholly ignored or misleadingly appropriated by PN proponents. This (mis)use of history, we argue helps to sustain the hype of the novelty of the proposed field and potential commodification of molecular advice that undermines longer histories of food management in premodern and non-Western cultures. Moreover, it elides how the longer history of nutritional advice always happened in a heavily moralized, gendered, and racialized context deeply entwined with collective technologies of power, not just individual advice. This article aims at offering a wider appreciation of this longer history to nuance the hype and exceptionalism surrounding contemporary claims.

To complicate the existing problem there are no dependable hepatoprotective drugs and a large number of patients prefer using complementary and alternative medicines for treating and managing hepatic complications. Almost 25 centuries ago, Hippocrates, the father of medicine, proclaimed "Let food be thy medicine and medicine be thy food." Exploring the association between diet and health continues even today. Preclinical studies carried out in the recent past have shown that the commonly used dietary agents like Allium sativum (garlic), Camellia sinensis (tea), Curcuma longa (turmeric), Emblica officinalis (Indian gooseberry), Ferula asafoetida (asafoetida), Garcinia cambogia (Malabar tamarind), Glycine max (soyabean), Murraya koenigii (curry leaves), Piper betle (beetle leaf), Prunus armeniaca (apricot), Ocimum gratissimum (wild basil), Theobroma cacao (cocoa), Trigonella foenum-graecum (fenugreek) and Vitis vinifera (grapes) protect against ethanol-induced hepatotoxicity. Mechanistic studies have shown that the beneficial effects of these phytochemicals in preventing the ethanol-induced hepatotoxicity are mediated by the antioxidant, free radical scavenging, anti-inflammatory and anti-fibrotic effects. The present review for the first time collates the hepatoprotective effects of these agents and also emphasizes on aspects that need future research to establish their utility in humans.

Numerous clinical, physiopathological and epidemiological studies have underlined the detrimental or beneficial role of nutritional factors in complex inflammation related disorders such as allergy, asthma, obesity, type 2 diabetes, cardiovascular disease, rheumatoid arthritis and cancer. Today, nutritional research has shifted from alleviating nutrient deficiencies to chronic disease prevention. It is known that lifestyle, environmental conditions and nutritional compounds influence gene expression. Gene expression states are set by transcriptional activators and repressors and are often locked in by cell-heritable chromatin states. Only recently, it has been observed that the environmental conditions and daily diet can affect transgenerational gene expression via "reversible" heritable epigenetic mechanisms. Epigenetic changes in DNA methylation patterns at CpG sites (epimutations) or corrupt chromatin states of key inflammatory genes and noncoding RNAs, recently emerged as major governing factors in cancer, chronic inflammatory and metabolic disorders. Reciprocally, inflammation, metabolic stress and diet composition can also change activities of the epigenetic machinery and indirectly or directly change chromatin marks. This has recently launched re-exploration of anti-inflammatory bioactive food components for characterization of their effects on epigenome modifying enzymatic activities (acetylation, methylation, phosphorylation, ribosylation, oxidation, ubiquitination, sumoylation). This may allow to improve healthy aging by reversing disease prone epimutations involved in chronic inflammatory and metabolic disorders.

The accuracy of quotations and references in six medical journals published during January 1984 was assessed. The original author was misquoted in 15% of all references, and most of the errors would have misled readers. Errors in citation of references occurred in 24%, of which 8% were major errors--that is, they prevented immediate identification of the source of the reference. Inaccurate quotations and citations are displeasing for the original author, misleading for the reader, and mean that untruths become "accepted fact." Some suggestions for reducing these high levels of inaccuracy are that papers scheduled for publication with errors of citation should be returned to the author and checked completely and a permanent column specifically for misquotations could be inserted into the journal.

The prevalenz of diverticulosis is similar in men and women. Diverticulosis increases with age (10% in adults about 40 years, more than 60% in older people). 25% will develop a symptomatic diverticular disease or diverticulitis. Epidemiological studies have clearly demonstrated associations between diverticulosis and diet, low in dietary fiber and high in refined carbohydrates. A low intake of fiber reduces the gastrointestinale transittime, increases the intracolonic pressure and may lead to the development of diverticulosis. More factors are well known to increase the risk for diverticulosis and diverticular disease as obesity, constipation and a reduced physical activity. But the most important factor is dietary fiber consumption. The nutrition should be poor in fiber or fiberfree respectively in case of acute diverticulitis. The fiber intake should be high (up to 30 gramms a day) in asymptomatic patients to prevent the development and complications of diverticulosis.

Extensive research in the past decade has revealed cancer to be a multigenic disease caused by perturbation of multiple cell signalling pathways and dysregulation of numerous gene products, all of which have been linked to inflammation. It is also becoming evident that various lifestyle factors, such as tobacco and alcohol use, diet, environmental pollution, radiation and infections, can cause chronic inflammation and lead to tumourigenesis. Chronic diseases caused by ongoing inflammation therefore require chronic, not acute, treatment. Nutraceuticals, compounds derived from fruits, vegetables, spices and cereals, can be used chronically. This study discusses the molecular targets of some nutraceuticals that happen to be markers of chronic inflammation and how they can prevent or treat cancer. These naturally-occurring agents in the diet have great potential as anti-cancer drugs, thus proving Hippocrates, who proclaimed 25 centuries ago, 'Let food be thy medicine and medicine be thy food'.

Reference inaccuracy in scientific articles brings the scientific validity of the research into question and may create difficulty when accessing the cited background data. The objective of this study was to examine the reference accuracy in the general surgery literature and its correlation with the journal impact factor. Five general surgery journals were chosen with varying impact factors. From the year 2007, one issue was randomly chosen from each journal, and from each issue 180 citations were randomly chosen for review. Three investigators evaluated the chosen references for primary, citational, and quotational errors. The impact factor of each journal was compared to the percentage of errors detected. The total number of errors per journal ranged from 31.3 to 39.3%, with a total of 35.4% of all citations reviewed containing some type of error. The most common error type detected was incorrect citation of the primary source supporting a statement, the incidence of which ranged from 13.8 to 25.2%, depending on the journal, and accounting for 53.6% of the total errors found. Citational errors, which included incorrect author names, pagination, dates, and issue and volume numbers, ranged from 1.8 to 18.1% and accounted for 20.4% of the total errors detected. Qualitative errors, which occurred when the author misquoted another author's written assertions or conclusions, ranged from 7.4 to 16.0% and accounted for 34.7% of the total errors detected. Quantitative errors (misquoted numerical data) ranged from 3.1 to 8.6% and accounted for 17.9% of the total errors detected. No association between impact factor and error rate was demonstrated. Reference inaccuracy is common in the general surgery literature. The impact factor has no clear association with the error rate, demonstrating that journal quality does not necessarily correlate with reference quality. Further investigation into potential methods for improving reference accuracy in the general surgery literature is warranted.

Acne is the most prevalent skin condition. It has a substantial effect on the quality of life of teenagers worldwide. Among acne patients from different societies and cultures, diet is uniformly regarded as a major cause in the pathogenesis of acne. We reviewed the up-to-date literature regarding acne and culprit foods such as dairy products, chocolate, and fatty foods. Unfortunately, after reviewing the existing data, there are no answers but there are definitely more questions, because there is no clear proof about whether these issues are facts or misconceptions.

Epidemiological and experimental studies have revealed that a mild to moderate drinking of wine, particularly red wine, attenuates the cardiovascular, cerebrovascular, and peripheral vascular risk. However, the experimental basis for such health benefits is not fully understood. The cardioprotective effect of wine has been attributed to both components of wine: the alcoholic portion and, more importantly, the alcohol-free portion containing antioxidants. Wines are manufactured from grapes, which also contain a large variety of antioxidants, including resveratrol, catechin, epicatechin, and proanthocyanidins. Resveratrol is mainly found in the grape skin, whereas proanthocyanidins are found only in the seeds. Recent studies have demonstrated that resveratrol and proanthocyanidin are the major compounds present in grapes and wines responsible for cardioprotection. The purpose of this review is to provide evidence that grapes, wines, and resveratrol are equally important in reducing the risk of morbidity and mortality due to cardiovascular complications. Both wines and grapes can attenuate cardiac diseases such as atherosclerosis and ischemic heart disease. Recently, wine was also found to increase life span by inducing longevity genes. It appears that resveratrol and proanthocyanidins, especially resveratrol, present in grapes and wines play a crucial role in cardioprotective abilities of grapes and wines.
