MULTILAYER PERCEPTRON CLASSIFICATION MODEL FOR DETECTING EMOTIONAL DISTRESS IN BREAST CANCER PATIENTS

Abstract

Breast cancer is one of the most common malignancies, and one of the leading causes of cancer-related death among women. This research aimed to explain emotional distress based on self-empowerment skills and interpersonal interactions in breast cancer patients using the explainable multilayer perceptron classification model. The participants in this research were a sample of 735 patients age ranged from 19-80 with breast cancer from Tajrish Hospital in Tehran in 2022 who were selected by convenience sampling through online platforms. The survey assessments included demographic characteristics (age and job, marital status and educational level), emotional distress, self-empowerment skills, interpersonal interactions, and family care. The data were analyzed using the Multilayer Perceptron Classification Model. The input layer includes age and job, marital status and educational level, self-empowerment skills, interpersonal interactions, family care and gender) and the output layer (target feature) includes emotional distress divided into low and high groups using a mean split. The results indicated that the accuracy of the classification in the training and test samples was 75.6% and 73.5% respectively. The AUC of 0.812 in the models was near to 1 indicating an excellent fit. Interpersonal interactions, self-empowerment, family care, age, having job, educational level and marital status were the most important effects in explaining the classification of emotional distress. MULTILAYER PERCEPTRON CLASSIFICATION MODEL FOR
DETECTING EMOTIONAL DISTRESS IN BREAST CANCER PATIENTS

ABSTRACT
Breast cancer is one of the most common malignancies, and one of the leading causes of
cancer-related death among women. This research aimed to explain emotional distress based on
self-empowerment skills and interpersonal interactions in breast cancer patients using the
explainable multilayer perceptron classification model. The participants in this research were a
sample of 735 patients age ranged from 19-80 with breast cancer from Tajrish Hospital in Tehran
in 2022 who were selected by convenience sampling through online platforms. The survey
assessments included demographic characteristics (age and job, marital status and educational
level), emotional distress, self-empowerment skills, interpersonal interactions, and family care.
The data were analyzed using the Multilayer Perceptron Classification Model. The input layer
includes age and job, marital status and educational level, self-empowerment skills, interpersonal
interactions, family care and gender) and the output layer (target feature) includes emotional
distress divided into low and high groups using a mean split. The results indicated that the
accuracy of the classification in the training and test samples was 75.6% and 73.5% respectively.
The AUC of 0.812 in the models was near to 1 indicating an excellent fit. Interpersonal
interactions, self-empowerment, family care, age, having job, educational level and marital status
were the most important effects in explaining the classification of emotional distress.

INTRODUCTION
Breast cancer is a prevalent and serious health concern affecting millions of women
worldwide. It is among the most common malignancies and is one of the leading causes of cancer-
related death among women. Breast cancer develops when abnormal cells in the breast grow
uncontrollably, and if left untreated, can spread to other parts of the body. It is essential to diagnose
and treat breast cancer as early as possible to increase the chances of successful treatment and
improve patient outcomes. Breast cancer can cause significant emotional distress in patients. The
diagnosis alone can be a traumatic and life-changing event, leading to fear, anxiety, and
depression. Patients may experience feelings of shock, disbelief, anger, guilt, and helplessness. The
uncertainty of the future and the potential impact on family and relationships can add to the
emotional burden. Treatment, including surgery, radiation, and chemotherapy, can also be
physically and emotionally taxing, leading to further distress. The fear of recurrence and the
potential for long-term side effects can also contribute to ongoing emotional challenges. It is
essential that healthcare providers address the emotional needs of breast cancer patients to support
their overall well-being and quality of life.
Several studies have investigated the emotional distress experienced by breast cancer patients.
For instance, a study by Grégoire et al. (2017) present results showed that mindbody interventions
seem to be an interesting psychological approach to improve the well-being of breast cancer
patients. Ouakinin (2015) described a predictive model which suggests the impact of pattern of

anger management and an anxious attachment style in the emotional disturbance reported by these
patients. Findings suggest that insecure attachment schemes, dysfunctional anger regulation
strategies and a lack of psychophysiological activation may be discussed as relevant factors that
modulate emotional distress associated with the diagnosis of breast cancer. Cohen (2002)
highlighted the importance and relevance of positive attitude for reducing emotional distress in
general and level of anxiety in particular. Saeedi-Saedi (2015) concluded that oncology specialists
should utilize mental health services to improve their patients’ mental health as well as to control
the consequences of the disease because emotional distress can affect the quality of life of breast
cancer patients. Quattropani, Lenzo, Mucciardi & Toffle (2016) Stated that gender, negative beliefs
and cognitive self-consciousness explained the 58% variance for overall distress of patients during
chemotherapy.
Overall, these studies suggest that emotional distress is a significant issue among breast
cancer patients, and interventions that address their emotional needs can be beneficial.
This goal of research is explaining emotional distress based on self-empowerment skills and
interpersonal interactions in breast cancer patients using the explainable multilayer perceptron
classification model.
METHODOLOGY
The participants in this research were a sample of 735 patients age ranged from 19-80 with
breast cancer from Tajrish Hospital in Tehran in 2022 who were selected by convenience sampling
through online platforms. The survey assessments included demographic characteristics (age and
job, gender, marital status and educational level), Emotional Distress, Self-Empowerment Skills,
Interpersonal Interactions, and family care. The data were analyzed using the Multilayer
Perceptron Classification Model. The input layer includes age and job, marital status and
educational level, Self-Empowerment Skills, Interpersonal Interactions, and family care) and the
output layer (target feature) includes Emotional Distress divided into Low and High groups using a
mean split. For increasing the accuracy of the model and avoiding overfitting, two hidden layers
were determined consisting of 12 and 10 units respectively. Activation functions used were the
Hyperbolic tangent and Sigmoid in the hidden layer and output layer respectively. 70% of the total
sample was used as the training sample and the rest was used as the test sample. Table 1 presents

RESULTS AND DISCUSSION
The data were analyzed using the Multilayer Perceptron Classification Model. The input layer
includes 7 features age and job, marital status and educational level, emotional distress, self-
empowerment skills, interpersonal interactions, and family care) and the output layer (target
feature) includes emotional distress divided into low and high groups using a mean split.

Table 2 presents a summary of processing. There are 735 valid instances for classification.
Table 3 presents main results of research. There are presented correct percent for low and
high emotional distress for training and testing. The overall percent for testing phase is 75%, which
is satisfactory for the observed theme.

Figure 2 presents sensitivity and specify and also lift chart. Both parts present low and high
emotional distress and their specific characteristics during classification process. Sensitivity
measures how well the model can correctly identify instances of the positive class, while
specificity measures how well the model can correctly identify instances of the negative class.
Table 4 presents the importance of independent variables. The highest importance is noticed at
Interpersonal interaction and self-empowerment, respectively. Visualization of proposed results is
shown at Figure 3.

CONCLUSIONS
The findings of this study revealed that the classification model achieved an accuracy of
75.6% in the training sample and 73.5% in the test sample, indicating a reasonably good
performance. The area under the curve (AUC) value of 0.812 suggested an excellent fit of the
model. The results further indicated that interpersonal interactions, self-empowerment skills and
family care were identified as the most significant factors influencing the classification of
emotional distress in breast cancer patients. These findings highlight the importance of considering
psychosocial factors such as self-empowerment skills and interpersonal interactions in
understanding and addressing emotional distress in breast cancer patients. By recognizing the

influence of these factors, healthcare professionals can develop targeted interventions and support
strategies to enhance the emotional well-being of individuals affected by breast cancer. Future
research could benefit from larger and more diverse samples to ensure broader generalizability of
the findings.
