How is the role of cortical bone density which is affected by osteoporosis, in bone fracture mechanism?

Bone fracture is the most common effect in osteoporosis. one the reason is the impact of osteoporosis on the density of the cortical bone. So I'm looking for the role of the density disorder which is caused by osteoporosis in bone fracture.

It's clear, osteoporosis has a direct impact on bone density. How can I figure out the manner of this effect in spongy and cortical bone, separately?

For the placement of screws and pins to stabilize complex bone fractures, some surgical procedures apply the drilling of holes through the cortical bone. Do surgeons make use of drill guides, and does anyone know which drilling angles can be applied without slipping sideways over the bone surface?
Any cues related to tools and procedures are very welcome.

Spongy bone, also known as callousness bone or trabecular bone, is a very porous type of bone found in animals.
Cortical bone is the dense outer surface of bone that forms a protective layer around the internal cavity. This type of bone also known as compact bone makes up nearly 80% of skeletal mass and is imperative to body structure and weight bearing because of its high resistance to bending and torsion.

I am a anatomist and orthopaedic surgeon. 
CT osteoabsorptiometry has been famous for the method to asses the subchondral mineralization as a measure of stress loading to joint cartilage (Momma, AJSM 2011, Funakoshi AJSM 2016). However, I am not sure whether these methods could be apply to measure not a loading stress but a tensile stress from tendons or ligaments around articular joints.
Is there any methods or theories to asses the tensile stress from tendons or ligaments with the distribution of the subchondral mineralization around joints by using CT osteoabsorptiometry or other instruments.
Are there studies examining the microstructure of mammalian skulls?

While the long bones like femur have been extensively studied to understand bone adaptation, they have many muscle attachments which distort any interpretation of stress fields carried out previously. Skulls on the other hand behave like pressure vessels and therefore have a unique stress state.
