Background: Immediate implant placement in the maxillary anterior region was challenging, especially with the jumping gap and limited primary implant stability. Objective: To assess the autogenous dentin graft biomaterial's ability to improve the esthetic outcomes of an immediately inserted implant. Methods: Twenty patients with non-restorable retained roots in the maxillary non-molar region surrounded by natural sound teeth were included in this study after a complete clinical and radiological examination, including patient health and clinical fitness for the immediate dental implant procedure. A single dental implant was inserted for each patient, and the resulting jump distance was filled with the mineralized dentin graft and covered by gel foam with the figure of eight suturing. The Pink Esthetic Score index of the implant-supported prostheses was evaluated six months after the initial surgery. Results: Twenty eligible patients were included in this study. The assessment of the clinical measurements of the alveolar ridge width and gingival biotype before surgical intervention and after 6 months showed no significant differences between them. The mean Pink Esthetic Score (PES) was 7.75. Only two patients achieved a total esthetic score of 10; another three patients had a score of 8, four with a score of 7, and the rest with a score of 6. Conclusion: An autogenous dentine graft effectively achieves acceptable esthetic results with an isolated immediate dental implant inserted in the anterior maxillary area.

Although the autograft is regarded as the gold standard bone graft, β-TCP artificial alloplastic is widely used in bone defect healing and has been accepted in human and animal studies to be resorbed and interchanged by vital bone. The active form of vitamin D3 is calcitriol, on mesenchymal stem cells (MSCs) it has a direct influence on encouraging their osteogenic differentiation which is identified as osteoinductive. The need for osteoinductive ability is a demanding issue regarding medical and dental care. RUNX2 regards an essential transcriptional factor for osteoblast differentiation. Adult male New Zealand rabbits (Twenty-four) with an average weight of (1.5-2 kg) were elected in this study. Four holes were made intra-bone in both tibias of each animal. The experimental groups divide as follows: Group C: Bone defect will be left to heal naturally as control; Group TCP: Bone defect will be occupied with β-TCP. Group vitamin D3: Bone defect will be filled with vitamin D3; Group TCPD: Bone defect will be filled with a combination of β-TCP and vitamin D3. Animals sacrificed at (1 and 3 weeks). Purification of total RNA from formalin fixed paraffin embedded (FFPE), The cDNA of RUNX2 and VDR amplify in RT-PCR and detected with sybr green. One-way Analysis of Variance (ANOVA) and least significant difference (LSD) was used as a tool for statistical analysis. Results at 7 days highest mean value of mRNA RUNX2 gene expression Fold Change estimated in TCP followed by TCPD experimental group while at 21 days, the highest mean value in TCPD. All measured parameters showed highly significant differences in the course of healing periods from 7 days to 21 days. In mRNA VDR gene expression parameters showed a highly significant difference in 7days and 21 days durations among experimental groups and the highest mean value of mRNA VDR gene expression Fold Change is recorded in TCPD followed by vitamin D3 groups in both durations. These findings suggest positive inductive effects of local application of vitamin D3 combined with β-TCP as well as vitamin D3 on osteoblast cells and induced bone defect healing.

Dental implant is the most reliable method to replace missing teeth. Primary implant stability and bone density are variables that have long been considered to be essential to achieve predictable Osseo integration and long term clinical survival. Evaluation of primary stability could be achieved by several methods, including the Periotest M values.According to the delayed protocol, it was important to wait several months after tooth extraction before placement of the implants to allow alveolar bone healing. Among the advantages of immediate implant protocol compared to delayed protocol: reduced bone resorption of the extraction socket, avoidance of a second surgery, andshortening of the treatment time and soft tissue aesthetics.the study aims To evaluate the difference in primary stability of dental implants measured by Periotest M device between immediate and delayed treatment protocols according to bone density, jaws, gender and age. A total of (80) Iraqi patients, (35) males and (45) females, received (100) dental implants, age ranged from (20-59) years, they were divided into two groups as follows: group of (50) dental implants in the delayed treatment protocol and group of (50) dental implants in the immediate treatment protocol were placed in both jaws and distributed in anterior and posterior sites. Pre-surgical general examinations were done, the orthopantomograph was obtained for all the patients and the dental implant fixture was measured their primary stability by Periotest M device after complete installation in osteotomy site. Bone density was determined according to the typical anatomical location of bone density based on Misch criteria and then specified the bone type according to resistance to drilling by the surgeon's perception. The results show that Statistical analysis was highly significant difference in mean periotest values in delayed group among bone density types (D2, D3 and D4). Highest mean periotest values were found in anterior site of mandible (-4.200), in D1 bone density (-4.2), in males (-3.081), at age range (30-39) years which was (-3.193) in delayed group. Primary stability of dental implants measured by Periotest M device was higher in delayed treatment protocol group than immediate treatment protocol group, in mandible than maxilla, at delayed group in males than females; at age range (30-39) years and in bone density type 1 among the other types 2, 3 and 4.

Background: The best material for dental implants is polyetherketoneketone (PEKK). However, this substance is neither osteoinductive nor osteoconductive, preventing direct bone apposition. Modifying the PEKK with bioactive elements like strontium hydroxyapatite is one method to overcome this (Sr-HA). Due to the technique's capacity to provide better control over the coating's properties, RF magnetron sputtering has been found to be a particularly useful technique for deposition. Materials and methods : With specific sputtering conditions, the RF magnetron technique was employed to provide a homogeneous and thin coating on Polyetherketoneketone substrates.. the coatings were characterized by Contact angle, adhesion test, X-ray diffraction (XRD), atomic force microscope and Elemental Analysis with Energy Dispersive X-Ray (EDX) Results : indicated that strontium hydroxyapatite had successfully deposited onto the surface with significant improvement in the wettability value to provide a suitable environment for cell attachment, spreading, proliferation, and differentiation Conclusion: Coating PEKK with RF magnetron sputtering can provide homogeneous surfaces laying the groundwork for improving PEKK's potential bioactivity, such as surface wettability. Wetting qualities are critical in implantable materials and are used to predict future osseointegration success.
