We report a girl who had dilated cardiomyopathy (DCM) and failed to thrive since birth. Transvenous cardiac resynchronization therapy with defibrillator (CRT-D) was implanted when she was 9-year-old, and her body size increased drastically thereafter, although serial chest plain films showed that the implantable cardioverter defibrillator (ICD) lead was gradually stretched with loss of lead curvature. We concluded that transvenous CRT-D may be clinically feasible and clinically beneficial in children with DCM, though "more than optimal" lead curvature reserve should be evaluated in advance to avoid potential lead retraction and malfunction of the pacing system.
