... vasculitis (RV) is a severe complication of rheumatoid arthritis (RA), which like a primary necrotising vasculitis can affect any organ but characteristically presents with painful cutaneous ulcers and systemic inflammation. 1 2 Usually patients with refractory RA are affected who, therefore, had already undergone extensive immunosuppressive treatment. The prognosis is poor and most patients die from infectious complications, cardiac failure, or cerebral insult. 3–6 Cyclophosphamide (CYC) is the preferred treatment but is often not well tolerated and does not contain the synovitis. 3 We report the cases of three patients with RV who could not sufficiently be treated by CYC and steroids but responded very well to infliximab infusion therapy. A 48 year old male patient with longlasting RA was admitted because of general malaise, dyspnoea, oedema, pleural and pericardial effusions, increased creatinine and liver enzymes, leucocytopenia, thrombocytopenia, decreased C4 and CH 50 , and increased levels of circulating immune complexes. The diagnosis of RV was established. The patient’s condition did not sufficiently improve after treatment with steroid pulses, high dose CYC, or even plasmapheresis. He developed a life threatening heart failure caused by a “swinging heart” due to the pericardial effusion, which required immediate and repeated drainage (fig 1). As a last resort we decided to start infliximab treatment at 3 mg/kg. After the first infusion his condition improved rapidly and the pericardial effusion, in particular, disappeared within two weeks. All laboratory find- ings returned to normal. He continues to receive infliximab every eight weeks and is in good clinical condition. Even the activity of his RA, previously not sufficiently controlled by methotrexate alone, has decreased significantly. A 60 year old woman with a history of aggressive seropositive RA for 36 years suddenly developed painful ulcers on her left leg. The diagnosis of RV was established by biopsy from the ulcer rim. CYC bolus therapy was started and steroids also had to be increased. She responded partially to CYC, but the ulcers did not heal completely. Because of infections and leucopenia the dose and interval of the infusions often had to be adjusted. After 17 boli within 22 months the CYC therapy was stopped because of severe leucopenia. The lesions worsened after an ineffective trial of cyclosporin A (fig 2A). Infliximab was given at 3 mg /kg in weeks 0, 2, and 6 and thereafter every eight weeks. The ulcers improved soon after the second infusion and were completely healed after nine months (fig 2B). Her RA activity also improved greatly. A 64 year old male patient with RA was admitted to our hos- pital with a fistula between the colon and the urine bladder, which required immediate surgery because of imminent per- foration. After surgery the wound did not heal. In addition, painful ulcers appeared on both legs, the scrotum, and other skin areas. All biopsies including that from the fistula, revealed necrotising vasculitis. CYC bolus therapy was started. The scrotal ulcer healed and the belly wound gradually improved, but the leg ulcers remained unchanged and his RA activity increased sharply. CYC had to be stopped because of leucopenia. Infliximab was started at 3 mg/kg at weeks 0, 2, and 6, and thereafter every eight weeks. The treatment was immediately effective for his synovitis and after a while the belly wound and the leg ulcers healed also. These cases demonstrate a very rapid and sustained improvement not only of the vasculitic symptoms but also of the inflammatory joint activity, suggesting that infliximab may be a valuable alternative to standard CYC/steroid therapy for severe RV. ow dose methotrexate (MTX) is widely used in the treatment of rheumatoid arthritis (RA) and various rheumatic disorders, including juvenile idiopathic arthritis (JIA). MTX is a folate antagonist, and its main adverse effects, which include haematological and hepatic toxicities, are well known. Used in high dosages in paediatric oncology, MTX has been associated with an osteopathy which is characterised by bone pain, osteoporosis, and insufficiency fractures of the legs. 1 The occurrence of MTX osteopathy in patients treated with low dose MTX has been reported but is still debated. 2–4 A 36 year old woman presented with severe polyarthralgias lasting for the past two months. She had a 27 year history of polyarticular type JIA, and had received prednisone up to 10 mg/day for the past 25 years. She had no history of osteoporotic or insufficiency fractures. Physical examination showed multiple synovitis of hands, wrists, knees, and ankles. Laboratory investigations showed a slight increase of C reactive protein of 20 mg/l, and a strongly positive rheumatoid factor. Low dose oral MTX was started at an initial weekly dose of 7.5 mg (weight 56 kg, height 156 cm, albumin 42 g/l, serum creatinine 60 μ mol/l). In the absence of significant improvement, two months later, the weekly dose of MTX was increased to 10 mg. One month later, while she had a persistent active polyarthritis, and after having received a cumulative dose of 97.5 mg of MTX, she complained of sudden and spontaneous onset of right groin pain that was relieved by rest. Standard radiographs showed a fracture of both inferior and superior right pubic ramus. Serum calcium and 25- hydroxycholecalciferol levels were normal. Pain resolved with rest in a few weeks. Treatment with MTX was maintained. Two months later, the patient presented with bilateral leg pain increased by weight bearing and relieved by rest. At that time, the received cumulative dose of MTX was 137.5 mg. In patients with RA, low dose MTX treatment was not associated with increased bone loss in the lumbar spine or the femoral neck at three years. 7 However, among the patients who were also receiving prednisone ( > 5 mg/day), MTX use was associated with greater bone loss in the lumbar spine, suggesting that the addition of MTX to prednisone may cause more bone loss than would be expected from corticosteroid treatment alone. 7 Recently, Uehara et al have shown in vitro that MTX impairs bone for- mation by inhibiting the differentiation of osteoblast precursors. 8 atients with systemic sclerosis (SSc) may have arterial and venous thrombosis and, according to the limited and controversial data available, may have an increased incidence of pregnancy losses. 1 These observations preceded the definition of antiphospholipid syndrome (APS) as the association of thrombosis and pregnancy loss with antiphospholipid antibodies (aPL), and did not focus on patients with SSc. However, the association of thrombosis and aPL, detected as lupus anticoagulant (LAC) and/or anticardiolipin antibodies (aCL), although rare, was described in SSc, 2 supporting the possible existence of a “secondary” APS in SSc. 3 In view of the fact that most aCL are directed to β 2 glycoprotein I (a GPI), 4 the possibility that patients with APS In patients with inflammatory arthritis receiving corticosteroids, MTX treatment should be considered as an additional risk factor for stress fractures. As far as we know this is the first reported case of MTX osteopathy in a patient with JIA. Rheumatologists should be aware of this complication as it may be easily confused with synovitis. Involvement of the leg articular or periarticular area should raise diagnostic clinical awareness. A bone scan is particularly useful for the diagnosis. 3 may be negative for aCL, but positive for a 2 GPI, and consid- ering the scarcity of data examining this issue in SSc, we read with great interest the recent study by Schoenroth et al , 6 who examined the frequency of a β 2 GPI in SSc. The authors found IgM a β 2 GPI in 2/26 (8%) patients and IgG in none. This find- ing did not seem to be related to any clinical or laboratory features. In another report, 80 patients with SSc were studied using an enzyme linked immunosorbent assay (ELISA) detecting the complex cardiolipin/ β 2 GPI. A similar prevalence of aCL/ β 2 GPI (10% IgG and 6% IgM), was found and a significant correlation between the presence of aCL/ β 2 GPI IgG and isolated pulmonary hypertension. 7 Looking retrospectively at our cohort of 115 patients with SSc fulfilling the American College of Rheumatology criteria, we found that, where clinically indicated, both aCL and a 2 GPI had been routinely evaluated in 60 patients (four male, 56 female; mean age 57 years; mean disease duration 13 years, range 1–42). These patients were classified, according to Le Roy (1988), as having limited (lSSc; n=48) or diffuse SSc (dSSc; n=12). Twenty seven patients were anticentromere positive and 16 anti-Scl-70+. Anticardiolipin antibodies were evaluated by a routine standardised method, 8 and a β 2 GPI as described by Balestrieri et al 9 ; values higher than the 99th cen- tile of 100 healthy blood donors were regarded as positive. Positive tests for aCL were found in 8/60 (13%) patients and for a β 2 GPI in 14/60 (23%) (table 1). The prevalence of a β 2 GPI was higher than in previous studies, probably because we per- formed the test only where clinically indicated; therefore, the prevalence in patients with SSc overall may differ. Among 60 patients, eight had a history of documented venous (four) or arterial (four) thromboses: two were aCL+ a β 2 GPI+, two aCL+ a β 2 GPI − , one aCL − a β 2 GPI+, and three aCL − a β 2 GPI − ; aCL and anamnestic thrombosis were significantly related (p<0.01; χ 2 with Yates’s correction). Two patients had “primary” (that is, not secondary to lung fibrosis) pulmonary hypertension. One patient was aCL+ a β 2 GPI+, whereas the other one was aCL+ and LAC+, but a β 2 GPI − ; aCL and pulmonary hypertension were significantly related (p=0.02). According to the Sapporo criteria 10 three patients had a significant history of pregnancy loss without thromboses: one was aCL − a β 2 GPI+, but two were aCL − a β 2 GPI − . Objectives: Antiphospholipid antibodies (aPL) can be present in the sera of systemic sclerosis (SSc) patients. This study aimed to determine the prevalence of aPL in a cross-sectional study of SSc patients, to assess their clinical associations, to perform a systematic review of published reports and a meta-analysis to estimate the worldwide prevalence of aPL in SSc. Methods: Two-hundred and forty-nine SSc patients were consecutively tested once for lupus anticoagulant (LA), anticardiolipin (aCL), and anti-β2glycoprotein I (anti-β2GpI) antibodies. Clinical associations with aPL positivity were studied using a logistic regression model. A systematic review of the literature was carried out in PubMed and Embase. Meta-analysis was performed using number of aPL positive (at least one of the three antibodies positive) and negative patients. Meta-regression was used to study potential factors explaining the heterogeneity between studies. Results: In our cross-sectional study, aPL positivity was found in 16 patients (prevalence 6.4%; 95%CI [3.8–10.4]). In multivariate analysis, there was a significant association between aPL positivity and venous thrombosis (VT) (OR 6.25 [1.18–33.00]; p = 0.028) and miscarriage (OR 5.43; 95%CI [1.31–22.13]; p = 0.017). Twenty-four studies were included in the meta-analysis, representing a total population of 3036 SSc patients. The overall pooled prevalence of aPL in SSc was 14% (9–20) with a high degree of heterogeneity among studies. Conclusion: This study found a prevalence of aPL positivity in our SSc population of 6.4% (3.8–10.4) and an overall worldwide pooled prevalence of 14% (9–20). In our SSc population, aPL positivity was associated with VT and miscarriage. These data provide additional insights into the role of aPL in the vasculopathy observed in SSc.

The antiphospholipid syndrome (APS) can be primary, when it occurs alone, or secondary, when it is associated with another autoimmune disease, mainly systemic lupus erythematosus and rarely other autoimmune diseases. Cases described in literature (Medline 1966 to December 2009) associate the presence of antiphospholipid antibodies with the presence of APS and systemic sclerosis (SS). Currently, however, no cases of the SS variant sine scleroderma with APS have been described. In this study, the authors describe the case of a patient with APS characterised by thrombosis of the retinal veins, in May 2006, the presence of lupus anticoagulant and an anticardiolipin IgG antibody. In May 2007, this patient developed Raynaud's phenomenon, a lack of oesophageal motility and nailfold capillaroscopy with a scleroderma pattern. The patient was positive for the anti-centromere antibody but lacked any evidence of cutaneous thickening or involvement. In summary, the authors describe the first case of a patient with APS associated with SS sine scleroderma.

Systemic sclerosis (SSc) is thought to be an autoimmune disease, as autoantibodies against a variety of extractable nuclear antigens can be detected in patient sera. Subgrouping patients based on the type of autoantibodies present can be useful in diagnosis and management. Anti-centromere antibodies (ACA) and anti-topoisomerase I antibodies (anti-topo I) are the classic autoantibodies associated with SSc. ACA are associated with limited cutaneous involvement and isolated pulmonary hypertension, whereas anti-topo I are associated with diffuse skin involvement and pulmonary fibrosis. ACA are predictors for a favorable prognosis, while anti-topo I are correlated with a poor prognosis and SSc-related mortality. Additionally, anti-RNA polymerase antibodies (anti-RNAP) are associated with diffuse cutaneous disease and renal involvement. Anti-nucleolar antibodies define multiple subgroups of patients with SSc. Of these, anti-Th/To antibodies (anti-Th/To) and anti-PM-Scl antibodies (anti-PM-Scl) are associated with limited cutaneous SSc (lSSc), whereas anti-U3RNP antibodies (anti-U3RNP) are associated with diffuse cutaneous SSc (dSSc). In addition, anti-Th/To and anti-U3RNP can be predictors for a less favorable prognosis with a higher frequency of organ involvement, such as pulmonary fibrosis, pulmonary hypertension and renal crisis. Other autoantibodies are less frequently reported: anti-Ku antibodies, anti-U1RNP antibodies, anti-human upstream-binding factor, and anti-U11/U12 antibodies. These antibodies are generally less specific to SSc, but also define clinically distinct patient subsets. Thus, characterization of autoantibodies in SSc together with knowledge of disease characteristics intrinsic to distinct patient populations is helpful for assessing the clinical presentation and prognosis of this disease, and for monitoring patients with SSc.

Systemic sclerosis (SSc; scleroderma) is characterized by a unique widespread vascular involvement that can lead to severe digital ischemia, pulmonary arterial hypertension (PAH), or other organ dysfunction. Microthrombotic events and procoagulation factors such as anti-beta2-glycoprotein I (anti-beta2GPI) or anticardiolipin antibodies (aCL) may be implicated in the development of these manifestations. This study was undertaken to investigate whether anti-beta2GPI and aCL are correlated with macrovascular disease, including ischemic digital loss and PAH, in SSc patients. Seventy-five SSc patients with a history of ischemic digital loss and 75 matched SSc controls were evaluated. Anticentromere antibodies (ACAs), anti-beta2GPI, and aCL were measured, and clinical associations were determined using conditional and simple logistic regression models. Positivity for anti-beta2GPI was significantly more frequent in SSc patients with digital loss than in patients without digital loss (P=0.017), with the IgA isotype of anti-beta2GPI showing the strongest association (odds ratio [OR] 4.0). There was no significant difference in aCL frequency between patients with digital loss and control patients. After adjustment for demographic characteristics, disease type, smoking, and ACA, anti-beta2GPI positivity was significantly associated with active digital ischemia (OR 9.4), echocardiographically evident PAH (OR 4.8), and mortality (OR 2.9). ACA positivity was associated with history of digital loss (OR 3.28), but not with PAH or mortality. History of digital loss was strongly associated with increased mortality (OR 12.5). Anti-beta2GPI is significantly associated with macrovascular disease in SSc and independently predicts mortality. It is unclear whether it has a pathogenetic role or simply reveals the presence of underlying endothelial injury. The use of anti-beta2GPI as a biomarker of vascular disease in SSc should be further explored.

Systemic sclerosis is a generalized autoimmune connective tissue disease of unknown aetiology. Profound vascular and immunological dysregulations result in tissue fibrosis affecting the skin and internal organs. Currently, two main clinical subtypes are distinguished, i.e. limited and diffuse cutaneous systemic sclerosis, which differ significantly in the clinical course and prognosis. Autoantibodies against topoisomerase (Scl-70), centromere-associated proteins, and nucleolar antigens are important for the diagnosis of the disease and give clues for its clinical manifestations and prognosis (prognostic autoantibodies). For most of these antibodies, however, the role in pathogenesis is not established. Anti-centromere antibodies are associated with limited cutaneous involvement and risk for pulmonary hypertension, whereas anti-topoisomerase I is associated with diffuse progressive disease and severe interstitial lung disease. Anti-Th/To positivity is associated with limited skin involvement but a high risk for severe internal organ involvement (Kidneys, PAH, Lung fibrosis). Anti-RNA polymerase I/III antibodies are associated with a high risk for renal involvement. Autoantibodies against the PDGF receptor and fibrillin-1 seem to play important roles in the pathogenetic process of systemic sclerosis.

Scleroderma is an autoimmune disease involving endothelial cell damage and fibroblast overproduction of extracellular matrix. Several autoantibodies present in the sera of patients with scleroderma, including anti-endothelial cell, antifibroblast, anti-matrix metalloproteinase, and antifibrillin-1 antibodies, may directly contribute to disease pathogenesis. Scleroderma also is characterized by the presence of antinuclear and antinucleolar antibodies, which correlate with particular phenotypes. These include antitopoisomerase-I, anticentromere, antihistone, anti-polymyositis/scleroderma, anti-Th/To, anti-U3-small nucleolar ribonucleoprotein particle, anti-U1-small nuclear ribonucleoprotein particle, anti-RNA polymerase, and anti-B23 antibodies. Other antibodies classically associated with other autoimmune diseases, such as antiphospholipid, antineutrophil cytoplasmic, and antimitochondrial antibodies, also have been described in patients with scleroderma. This review will summarize the various autoantibodies associated with scleroderma, their putative pathogenic roles, and their phenotypic correlations.

The prevalence of auto-antibodies associated to pulmonary arterial hypertension in scleroderma patients was reviewed, based on reports cited in two major scientific databases. Data were collected on the following types of antibodies: antinuclear, anti-double-stranded DNA, anticentromere, anti-CENP-A, anti-CENP-B, anti-bicaudal D2, anti-nucleolar, anti-Scl-70 (anti-topoisomerase I), anti-topoisomerase II α, anti-RNP, anti-U1RNP, anti-U3RNP, anti-RNA polymerase III, anti-Th/To, anti-histone, antiphospholipid, anti-PmScl, anti-Sm, anti SSA (anti-Ro),anti SSB (La), anti-Ro52 (TRIM 21), anti-Ku, anti-B23, anti-RuvBL1, anti-RuvBL2, anti-fibrin bound tissue plasminogen activator, anti-endothelial cell, anti-phosphatidylserine-prothrombin complex, anti-endothelin-1 type A receptor, anti-angiotensin II type 1 receptor, anti‑carbonic anhydrase II, anti-fibroblast, anti-cyclic citrullinated peptide, anti-4-sulfated N-Acetyl-lactosamine, class I and II anti-human leukocyte antigen. Auto-antibodies were shown by different authors to be associated to this condition, with different prevalence values for each type of auto-antibody. Antinuclear antibodies, anti-centromere antibodies, antiphospholipid antibodies, anti-U3 RNP antibodies and anti-Th/To antibodies would appear to show a particularly important prevalence in scleroderma patients with pulmonary hypertension, appearing in about 8/10 (antinuclear), 1/ 2 (anti-centromere, anti-phospholipid), and 1/4 (anti-U3RNP, anti-Th/To) of patients. The available evidence points in the direction of a strong association between auto-immune mechanisms and pulmonary hypertension in the setting of scleroderma.

Objective: To evaluate the clinical relevance of antiphospholipid antibodies (aPL) in systemic sclerosis (SSc). Methods: A systematic search of EMBASE and PubMed databases from January 1983 to July 2016 was carried out according to PRISMA guidelines whereas Peto׳s odds ratio (OR) for rare events was used for the meta-analysis. Results: The pooled prevalence of participants positive for IgG and IgM anticardiolipin (aCL) antibodies was higher in SSc than controls (12.8% vs 1.6% and 7.8% vs 0.6%; p < 0.0001 for both) as was that of IgG and IgM anti-beta-2-glycoprotein-I antibodies (aβ2GPI) (6.1% vs 0.58%, p < 0.0001; 3.5% vs 0.3%, p = 0.001). The pooled prevalence of pulmonary arterial hypertension (PAH) was more common in SSc positive than negative patients for aCL (IgG/IgM combined) (26.5% vs 10.9%, p < 0.0001) whereas the pooled prevalence of renal disease (RD) was more common in IgG aCL positive than negative patients (36.3% vs 10.9%, p = 0.02). The pooled prevalence of thrombosis was higher in IgG aCL, IgM aCL, and IgM aβ2GPI positive than negative SSc patients (12.6% vs 1.4%, p < 0.0001), (15.1% vs 2.7%, p = 0.002) and (15% vs 0.78%, p = 0.009), respectively. The pooled prevalence of digital infarction/ischemia (DI) was higher in IgG aCL and IgM positive than negative SSc (52.8% vs 39.8%, p = 0.002) and (68.1% vs 29%, p = 0.07). Conclusions: A strong relationship exists between aCL and aβ2GPI of IgG/IgM isotype and SSc; patients positive for these antibodies are more likely to suffer from PAH, RD, thrombosis, and DI. However, data expressed as frequency of aPL positive patients rather than average antibody titers preclude further insight into the relevance of these assumptions.

Systemic sclerosis is a connective tissue disease characterized by heterogeneous clinical picture, course and prognosis. It is hard to evaluate severity of the disease in individual clinical cases. There are not distinct phases of disease flare and remission. Lesions resulting from the inflammatory process (active disease), and from fibrosis (irreversible damage), can occur. Introduction to clinical practice of new biomarkers could be helpful for determination of indications for intensive immunosuppressive treatment and evaluation of its efficacy.
