RESUMEN En neurocirugía es muy frecuente el uso de agentes tópicos hemostáticos (por ejemplo, Gelfoam ® , Avatine ® , Surgicel ®). A pesar de su buena uti-lidad, existen casos donde se correlaciona con un aumento del edema peritumoral durante el posoperatorio y se cree que éste es secundario a una reacción inflamatoria ocasionada por esos agentes. Presentamos el caso de un paciente masculino afectado por un meningioma que desarrolló edema peritumoral después de una resección tumoral en donde se utilizó Gelfoam ®. Se hizo una revisión bibliográfica de los posibles mecanismos fisiopatológicos implicados, prevención y manejo. ABSTRACT Adequate hemostasis in neurosurgery is of paramount importance in a daily practice. There is a variety of hemostatic agents (Gelfoam TM , Avati-ne TM , Surgicel TM) which have demonstrated to be safe and biocompatible. Inflammatory response have been described against hemostatic agents that increase the peritumoral brain edema (PBE) after surgery in a few cases. We report the case of a 54-year-old male with frontal meningioma and during surgery, Gelfoamä was used as hemostatic agent; despite he was previously and after surgery treated with steroids, three days after tumoral resection PBE was increased. We reviewed the literature looking for possible pathophysiological mechanism, prevention and treatment.
