The potential benefits of promotion of physical activity for whole populations and at-risk individuals have become a well-established agenda for public health with various meta-analyses that have suggested physical fitness to be an important predictor of morbidity and mortality in adults. Lifestyle behaviors, such as diet and physical activity, are modifiable risk factors associated with many noncommunicable diseases, and chronic pathologies and health condition, such as obesity. To date, many intervention programs targeting physical activity and dietary changes have had modest effects and their long-term effectiveness is not well established. Thus, public health researchers have begun to examine novel approaches to deliver behavior change interventions. Mobile and wireless technology (health) is a growing area in the prevention and management of obesity that holds potential to deliver health-related behavior change interventions.

Background Obesity and the state of being overweight are increasing steadily and becoming a global epidemic. Recent research reports 64% of the adult population as overweight in Europe and the USA. The social and economic impacts are increasing, and most of the rehabilitation programs, while effective in the short term, do not produce long-lasting results. An explanatory model from a behavioral perspective can describe the phenomena with the lack of sources of reinforcement related to healthful habits in a daily life context. Methods/design A randomized clinical trial combining single-subject studies and a four-arm group design will be conducted to compare the effect of the current standard in obesity treatment to Acceptance and Commitment Therapy (ACT) and wearable technology at different times, before starting intervention, at the end, and at follow-up visits of 3, 6, and 12 months measuring changes over time of physical activity and psychological well-being. Discussion The goal of this project, combining ACT and wearable technology, is to develop an effective intervention, efficient and sustainable, which even after discharge can provide adequate contingencies of reinforcement in the natural environment, integrating systematic measurements, continuous feedback, and individualized, values-based objectives. The intervention is aimed to provide a contingent reinforcement for healthful behaviors instead of reinforcing only the achievement of a significant weight loss. The aim of the project, combining Acceptance and Commitment Therapy and Wearable Technology, is to develop an effective, efficient and sustainable intervention able to provide a contingent reinforcement for healthy behaviors. The intervention is aimed to promote adequate healthy behaviors in the natural environment, integrating systematic measurements, continuous feedback and individualized values-based objectives, instead of reinforcing only the achievement of a significant weight loss. SPLENDID is a research programme that develops a novel preventive intervention for young people at risk for obesity and eating disorders. The SPLENDID app, a novel smartphone application that mediates the monitoring and modification of the participants’ eating and activity behaviors, resides in the intervention’s core. The app receives and manages eating and physical activity related signals from three communicating sensors as well as subjective user input. In this paper, we present two discrete meal registration mechanisms – subjective and objective – that have been implemented and incorporated in the SPLENDID app, along with the relevant user feedback. In objective meal registration, the app records meal information with the help of a portable food weight scale, while an electronic meal report is employed for the subjective registration. Certain components of the proposed registration mechanisms and the relevant feedback have been evaluated with respect to usability on forty young adolescents, yielding promising results.

In the last two decades, whole-body vibration training (WBVT), involving exercising on a vibrating platform, emerged as an alternative exercise modality for the treatment of obesity. In this chapter, the possible clinical use of WBVT in obese individuals is addressed, involving its effect on body composition, muscle strength, and cardiovascular function.

The aim of this chapter is to present a practical overview of the most common equipment for patient handling and rehabilitation technologies for a clinical setting, focusing the attention on devices suited for obese individuals. In details, the equipment, devices, aids, and resources designed as alternative to manual handling are described. We have reviewed the equipment related to lifting, transferring, repositioning, moving, and mobilizing of obese patients ensuring that patients are cared for safely preventing consequences of immobility, while maintaining a safe work environment for employees.
