Abstract Melasma is one of the most common aesthetic problems that affect patients’ quality of life. The purpose of this study is to evaluate the efficacy of melasma treatment with a combination of 1064 PICO laser and 3% tranexamic acid compared with 3% tranexamic acid alone. This study is a prospective, randomized, assessor blinded study of volunteered melasma cases. The patient's faces were divided into 2 sides and were randomly treated with PICO laser and tranexamic acid on one side and only with tranexamic acid on the other. PICO laser sessions were conducted 3 times in a 4-week interval and the tranexamic acid was applied twice daily for 12 weeks. A total number of 21 patients were included in this study. The result of mMASI score and melatonin index in the combination of PICO and tranexamic acid group is statistically lower compared with the other group at 8 weeks and 12 weeks of follow up. However, the result shows that both groups had statistically significantly improved in mMASI score and melatonin index after 12 weeks of follow up. No statistically significant difference in patient satisfaction at 12 week of follow up and no adverse effects were observed. Keywords: melasma, picosecondlaser, tranexamic acid
